MINING ACTIVITIES WERE BEING CONDUCTED IN THE FACE OF THE NO. 1 ENTRY INBY AND ADJACENT TO THE OPEN CROSSCUT JUST DEVELOPED BETWEEN NO. 1 AND NO. 2 ENTRY THE LAST CUTIN THIS CROSSCUT WAS NOT B OLTED AND THE CROSSCUT OPENING IN THE NO. 1 ENTRY WAS NOT TIMBERED OFF. A ROOF FALL OCCURRED BEGINNING IN THE CORSSCUT AND EXTENDING OUT INTO THE NO. 1 ENTRY BETWEEN THE BOLTS WITH THE SECTIO
