THE EMPLOYEE CO4PLETED THE INSTALLATION OF A MECHANICAL ROOFBOLT AND WAS STANDING UNDER SUPPORTED TOP WHEN HE WAS STRUCK BY A PIECE OF COAL AND GRAY SHALE MEASURING APPROX. 3-1/2FT X 2-1/2 FTX 6 INCH THICK. 7HE MATERIAL FELL FROM A SLICKEN-SIDED SLIP LOCATED BETWEEN THE BOLTS OF A PROPERLY SUPPORTED AREA. THE EMPLOYEE SUFFERED A CRUSHING INJURY TO HIS HEAD.
