COAL WAS BEING 5OADED FROM THE NO.2 PILLAR OF THE 001 WORKING SECTION. I,NAME WAS STANDING AT THE CROSSCUT BETWEEN THE NO.2 AND 3 ENTRIES OBSERVING THE LOADING CYCLE WHEN THE ROOF COLLAPSED. T HE VICTIM WAS C2VERED BY THE ROOF FALL AND THE LOADING MACHINE OPERATOR WAS TEMPORARILY TRAPPED BENEATH THE CANOPY OF THE JOY 14 BU-10 LOADING MACHINE.
