EMPLOYEE WAS IN8TALLING ROOF BOLTS WITH A 300 GALIS ROOF BOLTING MACHINE AFTER BOLT WAS INSTALLED A LARGE ROCK THAT THE ROOF BOLT HAD BEEN INSTALLED IN FELL AND CAUGHT THE EMPLOYEES HEAD BETWE EN THE ROCK AND3THE DRILL HEAD OF THE ROOF BOLTING MACHINE
