SERVICE TRUCK W5S PARKED WITHOUT SETTING THE EMERGENCY BRAKE THE TRUCK WAS NOT LEFT IN GEAR, AND WHEEL CHOCKS WERE NOT USED. NONE OF THESE PRECAUTIONS WERE TAKEN ALLOWING THE TRUCK TO ROLL BA CKWARD PUSHING 9MPLOYEE INTO THE SCREEN PLANT
