EMPLOYEE WAS DRIVING A MANTRIP DOWN A GRADE OF 9.8 PERCENT WITHOUT WORKING BRAKES TWO TENTHS OF A MILE DOWN THE ROAD HE CLIMBED THE HILLSIDE FOR THE THIRD TIME TRYING TO SLOW THE MANTRIP DOWN THE MANTRIP ROLLED ON ITS SIDE WITH THE EMPLOYEE UNDERNEATH WHICH CAUSED HIS DEATH
