EMPLOYEES WERE REMOVING THE WHEELS AND AXLE FROM AN END LOADER, THE LOADER WAS BLOCKED UP WITH WOOD BLOCKS. THE END LOADER FELL OFF THE BLOCKS WHEN IT FELL THE BUCKET STRUCK THE EMPLOYEE, CRUS HING HIM.
