EMPLOYEE WAS ADJUSTING 5 1/2 FT STANDARD NORBERG CRUSHER WITH A 1" DIAMETER 100FT LONG WIRE ROPE ATTACHED TO A 988-B LOADER. THE CABLE WAS ATTACHED TO THE CRUSHER BY MEANS OF A 5" CLEVIS WITH A 5" LONG 1 1/4" DIAMETER BOLT. WHEN PRESSURE WAS APPLIED TO THE CABLE, THE 5"BOLT CAME OUT OF THE CLEVIS CAUSING WHIPPING EFFECT STRIKING HIM COMPLETELY SEVERING BOTH LEGS.
