THE DUST COLLECTION HOSE STOPPED UP. THE OPERATOT LEANED UP OVER THE DRILL POT AND LIFT ARMS TO REACH THE HOSE IN AN ATTEMPT TO UNSTOP IT. HIS LEG OR HIP CAME IN CONTACT WITH THE LEVER WHICH R AISES THE DRILL POT, CRUSHING HIS HEAD AND CHEST AGAINST THE ROOF RESULTING IN THE INJURY.
