A ROCK FALL OCCURRED ON 003 SECTION IN THE NO. 5 ENTRY, 50 FT INBY SPAD NO. 2838. THE CONTINOUS MINER WAS EXTRACTING COAL IN THE 5 RIGHT INTERSECTION WHEN A SUDDEN ROCK FALL FELL FATALILY INJ URY SECTION FOREMAN. WHO WAS ALONG SIDE OF MACHINE THE FALLW AS APPROXIMATELY 24 FOOT X 34 FOOT X 1 FOOT TO T 1/2 FOOT THICK.
