THE CONTINUOUS MINER, SIMMONS-RAND-MODEL 500 WAS BACKING OUT OF THE NO. 7 INTERSECTION-001-0 SECTION, AND THE VICTIM WAS HANDLING MINER CABLE ON THE RIPPER HEAD WHEN A LARGE PIECE OF MINING RO OF (SLATE,APP. 2 FT.X 4 FT. X 7", FELL BETWEEN THE RIGHT RIB BOLT AND RIB, STRIKING VICTIM.
