TRANSFORMER GIVING FALSE READING ON THE GROUND PHASE OF THE SWITCH. THE HAND-HELD MOMENTARY PUSH BUTTON WAS APPARENTLY MISSING DURING THE TIME HE WAS USING IT. WHEN HIS THUMB CONTACTED THE ENE RGIZED MECHANISM HE WAS ELECTROCUTED.
