Servicing equipment, hit forehead on part of truck at about 1100.He called EE,EE asked him how he was.EE said fine didn't need to go to the doctor. Our Supt.called EE about 2:00 PM from Garza County Health Clinic .He said EE was servicing loader and raised up and hit his back. Our supt. took him for medical treatment.

Employee was asked to pull up, the dump truck started rolling forward, EE tried to put the truck in reverse, hit the brakes, jumped out of the dump truck. Medical Examination and testing revealed only bruises. Excused from work 7/12/2006 through 7/15/2006. Released to return to work on 7/16/2006.
