TRANSFORMER GIVING FALSE READING ON THE GROUND PHASE OF THE SWITCH. THE HAND-HELD MOMENTARY PUSH BUTTON WAS APPARENTLY MISSING DURING THE TIME HE WAS USING IT. WHEN HIS THUMB CONTACTED THE ENE RGIZED MECHANISM HE WAS ELECTROCUTED.

Our Supt.called EE about 2:00 PM from Garza County Health Clinic .He said EE was servicing loader and raised up and hit his back. Our supt. took him for medical treatment.

Employee was asked to pull up, the dump truck started rolling forward, EE tried to put the truck in reverse, hit the brakes, jumped out of the dump truck. Medical Examination and testing revealed only bruises. Excused from work 7/12/2006 through 7/15/2006. Released to return to work on 7/16/2006.
