AFTER BLASTING TO REMOVE LOOSE ROOF IN THE MINE, THE EMPLOYEE ENTERED AN AREA THAT HAD NOT BEEN CLEARED OR INSPECTED FOR WORK DUTIES. THE LOOSE ROCK FELL ON HIM, FATALLY INJURING HIM. THE EMPO LOYEE WAS WAITING FOR THE AREA TO BE CLEARED OF THE SHOT.
