THE FOLLOWING WAS A CONCLUSION DRAWED FROM ACCIDENT SITE. THE EE WENT INTO HEADING TO BOLT TOP WHEN HE WENT INBY AN OPEN CROSSCUT NOT SUPPORTED HE PARKED HIS BOLTER AT THE LAST ROW OF BOLTS AT THAT TIME HE WENT INBY ROOF SUPPORTS APPROX. 30-40 INCHES ON MARKING 3RD BOLT. A PIECE OF DRAW ROCK APPROX. 15X18X8 FELL STRIKING HIM AND PINNED HIM AGAINST BOLT MACHINE.
