WORKER OPERATING FRONT END LOADER TO PLACE MATERIAL FOR A BERM NEXT TO AN OPEN PIT 12' DEEP, GOT TOO CLOSE AND THE PIT WALL COLLAPSED OVERTURNING THE LOADER AND KILLING THE OPERATOR.
