EE WAS WORKING IN A MANBASKET BEING LIFTED BY A BOOM CRANE, WHEN CABLE FROM CRANE SUPPORTING MANBASKET BROKE DROPPING BASKET AND CRANE SHEAVE BLOCK. CRANE SHEAVE BLOCK WEIGHS APPROX. 400 LBS A ND DROPPED ON THE EE CAUSING FATALITY.
