TEFLON INSERT BROKE OUT OF CONCRETE COUNTERWEIGHT WHILE EE WAS INSTALLING IT. COUNTERWEIGHT BROKE FREE FROM CRANE, THROWING EE TO THE GROUND. EE WAS KILLED INSTANTLY.
