EE WAS OPERATING AN EXCAVATOR LOADING COAL ONTO A 350B HAUL TRUCK WHEN SUDDENLY THE SLOPED HIGHWALL ABOVE THE EXCAVATOR GAVE WAY SLIDING INTO THE PIT COVEREING THE EXCAVATOR WITH MUD AND WATER , TRAPPING EE IN THE OPERATOR'S CAB, FATALLY INJURING HIM.
