THE MINER OPERATOR WAS IN PROCESS OF BACKING THE MINER OUT AFTER CUTTING THE RIGHT SIDE (HE HAD POSITION HIS BODY BETWEEN THE MINER AND THE RIB), SOMETHING CAUSED THE BACK OF THE MINER TO SLEW TO THE RIGHT AND THE VICTIM HEAD WAS CAUGHT WITH THE CABLE HOOK AND THE RIB.
