EMPLOYEE WALKE DOTU ON HEAD FRAME STRUCTURAL SUPPORT BEAMS WHILE INSTALLING COLLAR STRUCTURE. TURNING TO LEAVE SHAFT DOORS EMPLOYEE MISJUDGED WHERE HE WAS STEPPED OFF ON 3.5' RADIUS AIR SWEEP -LOSING HIS FOOTING AND FELL TO BOTTOM OF SHAFT.
