THE EMPLOYEE WAS LOCATED AT THE STAGELOADER MASTER CONTROLLER WHEN WITHOUT WARNING, RIB COAL AND ROCK BURST FROM THE PILLAR OUTBY THE LONGWALL FACE, STRIKING HER AND CAUSING FATAL CRUSHING INJ URIES.
