WHILE PREPARING TO POUR CONCRETE, CREW REPORTED ELEC PROBLEMS ON GALLOWAY. ELECTRICIAN SUMMONED.WHILE EN ROUTE, CONVEYANCE ENTERED CHAIRING PHASE ABOVE GALLOWAY. GUILLOTINE CROSSHEAD FAILED TO OPEN ON ONE SIDE. BUCKET FELL TO SHAFT BOTTOM FATALLY INJURING EMP BEFORE SLACK ROPE IDENTIFIED.EMP WAS ON BOTTOM DECK & PASSED THROUGH BUCKET WELL AT TIME BUCKET DROPPED.
