SUBCONTRACTOR'S EMPLOYEE WAS DRILLING BLASTING HOLES, WHEN DRILL HIT A VERTICAL SHAFT. HE FELL INTO THE SHAFT AND WAS BURRIED BY DEBRIS, AND WAS ASPHYXIATED.
