SUBCONTRACTOR'S EMPLOYEE WAS DRILLING BLASTING HOLES, WHEN DRILL HIT A VERTICAL SHAFT. HE FELL INTO THE SHAFT AND WAS BURRIED BY DEBRIS, AND WAS ASPHYXIATED.

CalPortland, Co. · Struck against a moving object

EE was operating a 988 loader, working on a road towards the east corner of the plug shot. EE was removing material, creating a pathway to connect to an existing ramp, for haul truck access. As EE pushed, material from the ramp on the front right side of the tire, succumbed to the weight of the loader and rollover. EE scrapped material at an angle from the ramp.

EE was removing a loader tooth when a sliver of metal from the loader tooth struck employee's neck. EE was evaluated on 3/9/2017 and given antibiotics.
