EMPOLOYEE WAS INSTALLING 6' SPLIT-SET ROCK BOLTS AND WIRE MESH IN THE LAST ROUND BLASTED WITH A JACKLEG DRILL. DURING THE BOLTING CYCLE, A MAJOR FALL OF GROUND OCCURRED AND EMPLOYEE WAS BURIED AND FATALLY INJURED.
