THE VICTIM HAD BEEN HAND-MUCKING AT THE BOTTOM OF THE SHAFT PRIOR TO SIGNALING THE SURFACE TO BE HOISTED OUT. THE SURFACE ATTENDANT DESCENDED THE SHAFT AFTER THE HOIST RIGGING ARRIVED AT THE S URFACE WITHOUT THE VICTIM ON IT. HE FOUND THE VICTIM UNCONSCIOUS, WITH NO VITAL SIGNS, NEAR THE BOTTOM OF THE SHAFT. THE AUTOPSY REPORT INDICATED IT WAS CARBON MONOXIDE POISONING.
