Amrize Canon City Quarry: 1 recorded fatality Metal/Non-Metal

Slipped and struck right arm against feed plate. Laceration to right arm, 6 stitches.

Employee was in a kneeling position and when they got up they stumbled off of a muck pile and came down on their left leg hurting their left knee.

Employee was cleaning out from under a catwalk and was struck on right shin by an attachment from the skid-steer that was assisting in the cleanup process, and received a contusion to the shin.

Employee was directing an asphalt truck back to dump on entrance road into quarry and was overcome with a massive heart attach (cardiac arrest)

EE was performing maintenance on crusher. Stepped backward and strained left knee. Examined at St. Thomas More, released without restrictions.

States that he was exiting haul truck in a hurry to get to the men's room. His foot slipped and he stopped his fall by grabbing the side rail with his left hand, spraining his left wrist. Steps were clean and dry.

Employee was cutting crown rubber with utility knife. Knife slipped and cut his left thumb. Stitches were required. Able to return to work.

Employee was cleaning cone for crusher where it is seated with airplane stripper. He was wearing safety glasses and long sleeve gloves at the time. Employee believes that after reaching into the cone with his glove to clear trash that he must have touched his face with his glove.

Was picking some wood out of rock and he twisted his right knee when he stepped wrong on a rock.

A driver of a leaser's truck caught his sneaker on the truck as he was getting down to get his scale ticket and he fell and hit the side of the scale house. He injured both his right shoulder and his right cheek.

Employee was rushing to help injured employee and hit right shin on handrail.

Employee was tightening bolt on crusher jaw. Bolt broke and wrench and spring hit him in the face. His nose was broken and stitches were required.

ROCK WAS PUSHED OFF HIGHWALL & HIT THE EDGE OF HIS MACHINE & JARRED HIM, INJURING HIS SHOULDER. (WC PENDING INVESTIGATION, EE DIDN'T REPORT TILL MUCH LATER.)

EE STATES THAT INJURY PROBABLY OCCURRED WHEN HE WAS LIFTING SOMETHING.

I NAME WAS USING A SLEDGE HAMMER TO POUND SOMETHING WHEN HE STRUCK WITH THE HAMMER HIS HAND MOVED DOWN THE HANDLE & STRUCK AGAINST THE OBJECT ALSO HE WAS WEARING GLOVES HE RECEIVED A SMALL CUT TO HIS RT INDEX FINGER I NAME SAW DR BECAUSE HETHOUGHT HE MIGHT NEED STITCHES DR ADVISED CLEANING TWICE DAILY APPLYING A SILVABENE CREAM & A REGULAR BANDAIDE

HE WAS BACKING THE AIR TRACK UP HE WAS STANDING AT THE SIDE WHEN HE STEPPED BACK HE TRIPPED ON A ROCK AND FELL HOLDING ONTO THE CONTROLS AND THE MACHINE RAN OVER HIS FOOT HE THEN REACHED UP TO THE CONTROLS AND MOVED THE MACHINE CRUSHED HIS R FOOT HE DIDNT HAVE ANY BROKEN BONES SINCE HE HAD ON SAFETY BOOTS BUT THE MUSCLE AND ARTERIES WERE CRUSHED

EMPLOYEE WAS HITTING TWO PIECES OF STEEL TOGETHER, AGAINST EACH OTHER AND SMASHED HIS THUMB BETWEEN THE TWO PIECES.

HE WAS HELPING TO GET SOME MATERIAL THAT WAS STUCK IN THE PRIMARY TO GO ON THROUGH,ANOTHER EMPLOYEE WAS USING A STEEL BAR TO SHOVE ON THE MATEIRAL WHEN A ROCK CAME FLYING OUT.

EMPLOYEE WAS CUTTING NUTS OFF OFF BELT FASTENERS WITH A WELDING TORCH AND WAS NOT WEARING THE CUSTOMARY EYE PROTECTION AND A PICE OF METAL GOT INTO HIS EYE.

HE WAS SET UP T2 START DRILLING WITH AN AIR TRAC DRILL WHEN HE DECIDED TO CLEAN SOME ROCK FROM UNDERNEATH THE STEEL, THE ROCK WAS LOOSENED AND THE STEEL CAME DOWN ON HIS FINGERS.

WHILE CLEANING ROCK OUT OF THE JAW, I NAME PUSHED ONE ROCK WITH ONE HAND ON TOP OF HIS OTHER HAND. WHICH CUT HIS FINGER.

WHILE EMPLOYEE WAS HELPING TO OTHER EMPLOYEES CHANGE SCREENS ON THE CRUSHER HE GOT DIRT IN HIS EYE THE DIRT WAS THOUGH TO BE WASHED OUT BUT A SMALL AMOUNT REMAINED AND THE EMPLOYEE KEPT RUBBI ING HIS EYE THEREFORE HE WENT TO THE DR AT THE END OF THE DAY TO THE REST OF THE DIRT OUT.

LIFTING OXYGEN TANK ONTO WELDING TRUCK

AT TIME OF ACCID HAD JUST HAD SERIOUS ACCID INVOLVING CONVEYOR TURNING OVER AND SERIOUSLY INJURING ONE AND RESULTING IN FATALITY TO ANOTHER- THIS EMPL WAS RUNNING TO HELP WHEN INJURED KNEE.

WHILE CHANGNG THE POSITION OF THE TIRES ON A STACKING CONVEYOR THE JACK SLIPPED AND THE CONVEYOR TURNED OVER SERIOUSLY INDURING ONE AND RESULTING IN A FATALITY FOR ANOTHER THE CONVEYOR WAS NOT PROPERLY BLOCKED OR CRIBBED

RAISING LOADER BUCKET WITH HOIST,BUCKET BECAME OVERBALANCED AND PINNED RIGHT LEG UNDER BUCKET. BROKE RIGHT LEG IN FOUR PLACES.
