The worker was reinstalling a spring onto a tension pulley system. During this process the worker was holding the pulley up with EE's right hand and installing a plate with EE's left hand, when the pulley slipped from EE's right hand and made contact with EE's left thumb.

Employee was in a kneeling position and when they got up they stumbled off of a muck pile and came down on their left leg hurting their left knee.
Employee was cleaning out from under a catwalk and was struck on right shin by an attachment from the skid-steer that was assisting in the cleanup process, and received a contusion to the shin.

States that he was exiting haul truck in a hurry to get to the men's room. His foot slipped and he stopped his fall by grabbing the side rail with his left hand, spraining his left wrist. Steps were clean and dry.
Employee was cutting crown rubber with utility knife. Knife slipped and cut his left thumb. Stitches were required. Able to return to work.
Employee was cleaning cone for crusher where it is seated with airplane stripper. He was wearing safety glasses and long sleeve gloves at the time. Employee believes that after reaching into the cone with his glove to clear trash that he must have touched his face with his glove.

EMPLOYEE WAS CUTTING NUTS OFF OFF BELT FASTENERS WITH A WELDING TORCH AND WAS NOT WEARING THE CUSTOMARY EYE PROTECTION AND A PICE OF METAL GOT INTO HIS EYE.

AT TIME OF ACCID HAD JUST HAD SERIOUS ACCID INVOLVING CONVEYOR TURNING OVER AND SERIOUSLY INJURING ONE AND RESULTING IN FATALITY TO ANOTHER- THIS EMPL WAS RUNNING TO HELP WHEN INJURED KNEE.
RAISING LOADER BUCKET WITH HOIST,BUCKET BECAME OVERBALANCED AND PINNED RIGHT LEG UNDER BUCKET. BROKE RIGHT LEG IN FOUR PLACES.
