Belt repairs being installed. Employee was cutting a v-shape cut at edge of belt and utility knife slipped, cutting hand.

Employee was replacing a screen panel when it slipped out of left hand and landed on right pinky finger, causing a puncture to right pinky finger.
Employee was replacing a guard on a tail pulley and strained shoulder and lower back.
Employee was attempting to free a screen tightening rail with a four foot bar and the rail came loose. The bar then came back and struck the employee on the left side of the face causing safety glasses to cut employee just above the eyebrow.

Employee was in the process of building a guard, and was welding at the time, when another employee with a lit cutting torch pulled on the torch to get more slack in the hose. In the process he ran the lit torch across the arm of the employee that was welding. This caused a second degree burn on the employee's upper left forearm about two inches in diameter.
