Employee was entering the wash tunnel. Employee lost their footing on staircase and fell forward impacting their hand on the concrete floor.

Employee was walking to the shop door which was locked so employee walked around to the south side of the shop and slipped and fell on ice hitting head and straining shoulder.
Employee was standing on a ground level catwalk, swapping out frozen water lines to the dust suppression system, when EE stepped backward rolling EE's ankle causing a fracture and dislocation to the ankle.
Employee attempted to manually roll and lift rock out of haul road. Employee reported to Employer injury after end of shift, employee sent for medical evaluation following morning.

Employee was removing the cap screw on the filter housing of the HP 300 cone crusher using a 36" aluminum pipe wrench. The wrench slipped off the nut causing the employees hand to strike a stationary object.
The operator of the base plant was standing on the guarded platform guiding the backhoe operator to place the rock breaker on the rock jam. The plant operator signaled the backhoe operator to swing to the left and it would not stop. The rock breaker struck the plant operator on the left cheek. EE fell down 4 stairs and injured.

A miner was exiting a MCC connex which consisted of 1 step down to ground level. The Miner mis-stepped on the level ground and rolled EE's ankle.

The Miner was assisting another miner carry a 80 pound screen bearing from a storage conex to a cart for transport to the work location. The bearing started to slip off the cart, as it was moving the miner stepped back to avoid being struck by the bearing. As they stepped back steel protruding from the conex struck their left leg just above the top of their boot.

A retest confirmed a Standard Threshold shift for employee in the left year
EE positioned company f-150 truck behind a reversing 980M loader without alerting or making any eye contact with the loader operator. The counterweight on loader struck the right front fender but pickup was drivable. No injury was reported by EE the day of the incident nor the next day. The first report of an injury by EE was late the next day. January 17,2017.

EE stood up from a knelt position while repairing a conveyor belt and felt a small amount of pain in right knee. EE then climbed into the ford F-350 service truck and once ee place heel on the floor board ee felt a pop in right knee.
Flashburn on eyes due to welding in truck bed
Gust of wind came up while employee was inspecting screens and dust got under their safety glasses and they got a small piece of debris in their eye.
The driver was trimming EE's load on an end dump trailer. EE closed the tailgate using the in cab controls and then went to the back tailgate to clear the seal so the tailgate could shut. EE placed EE's hand on the dog ear on the tailgate when the tailgate closed on EE's left hand crushing EE's hand and causing a laceration to EE's hand.

Employee was stepping down off of a ladder when he stepped on a rock causing his ankle to roll, spraining his ankle.
Individual was cutting flashing and the blade slipped, cutting the bottom part of the thumb. The cut required 3 stitches.
Employee was changing screens and got the tip of his right ring finger pinched underneath the screen railing, causing the finger to bruise and swell. Injury was reported and the employee was taken to Concentra to get finger looked at, x rays were taken and the tip of the right ring finger was broken.

The miner was installing screens on the bottom deck of a rinse screen. As he was swinging the hammer hit the cross member above deflecting the hammer striking his left middle finger causing a fracture to the tip of that finger. The impact of the hammer chipped the bone on the end of his finger.
Employee slipped on ice while working in the plant and injured his shoulder.

Employee, blaster, was dewatering blast holes, when he went to lower the dewatering pump into a blast hole a worn spot on the dewatering hose showing metal cords caught his middle right hand finger. He tore the skin in a v shape 1/2" by 1/2". Employee was sent to the clinic and had 5 stitches to close up the wound.

Employee was holding a piece of steel (crusher mainframe liner) underneath crusher #2. The piece of steel was held on to the mainframe by some welds. As the other non injured employee was cutting it off, the steel recoiled and pushed injured employee's arm back into his shoulder.
Employee was climbing a ladder. As he got to the 4th rung, the ladder slipped out from underneath him. He tried to grab a picking eye to regain balance but could not do so and slid to the ground.

Employee came into my office on the 2nd of Aug. complaining of irritation in his eye that happened on the 1st of Aug.
Replacing idler on CB2 tail section. As EE was tryng to line up the idler frame with the conveyor frame, the idler slipped off the frame hitting EE's finger.
Employee was kneeling on CBI, throwing rocks off. He lost balance and fell landing feet first.
Employee was lifting a 5 Gal container, shoulder height. As he lifted the container he felt a small twinge in his left forearm.
EE was stepping out of the cat 226B skidder with the bucket full of material and with the lip of the bucket tip up in the air. As he stepped out he tripped on the bucket and fell to the ground.
Ee was walking out of cb2 tunnel when he slipped on ice.

As employee was removing guards, he planted his foot on some mud/fines. His foot sank, quickly losing balance, shifting his momentum forward, resulting in employee striking the top of his hard hat against the plant structure.
EE was assisting another employee in removing bucket from skid loader. Bucket pins were stuck. Lifted bucker approx 6" in air & struck handle with hammer. Lower pin released, bucket swung forward, came off upper bucker receptical & swung forward approx 12" - 16" landing on ee's right foot.
Employee was pulling on a bar that became stuck. As he pulled back on the bar, it came loose and he trapped his thumb in between 2 bars.
EE called and informed us that he was going to see a doctor because he is having pain in his elbow. He stated that it was a persistant pain in his right elbow that has occurred over time, due to welding.
EE lifted large rock. Felt sharp pain in shoulder (left) approx. 1 hr. later.

EE was resting a 20 am motor on a handrail while still confected to the pully system, as employee tryed to move motor to the ground. The motor slipped off the rail pinching his hand in between the hand rail and the motor shaft.

THERE WAS NO BROKEN SKIN & EE FINISHED SHIFT. ABLE TO CONTINUE WITH WORK. THE FOLLOWING MONDAY ARM WAS STILL SORE & EE WAS SENT TO DR. WHO DIAGNOSED INJURY AS CONTUSION TO LEFT FOREARM.
SUPPORT GAVE WAY UNDER MACHINE. FINGERS GOT CAUGHT BETWEEN TIRE AND WHEEL SMASHING THE TIPS OF THREE FINGERS.
WHILE EE WAS STANDING ON HANDRAIL BESIDE SCREEN CHECKING SCRRENS FOR MUDE, HE SLIPPED AND FELL APPROX. 11' TO GROUND, LANDING ON HIS HEELS FRACTURING BOTH LEFT AND RIGHT HEEL.

EMPLOYEE WAS STANDING NEXT TO AN ELEVATED WALKWAY THAT HAD AND ELECTRICAL CABLE ATTACHED TO IT. WHILE ATEMPTING TO REMOVE THE CABLE FROM THE WALKWAY, SEVERAL OF THE TIES THAT ATTACHED THE CABL BE TO THE WALKWAY BROKE & ALLOWED THE CABLE TO STRIKE EMPLOYEE.
