EE reports hydraulic ladder pinched right leg between the handrail and the folding ladder. EE was assisting a third-party mechanic while the mechanic was troubleshooting the hydraulic ladder. Contacted WorkCare and after a few days of self-care, the EE was scheduled for a CV. EE was diagnosed with a torn ligament in right calf area.

An employee was conducting a pre-use equipment checklist when the employee slipped and fell from a water truck. The employee received sutures to the back of the head.

During the process of fueling the locomotive, the Locomotive Engineer received diesel splash back in the eyes from nozzle located at locomotive fuel island.

An employee was operating a CAT 777 Haul Unit in the L2 pit, when a boulder rolled out of the muck pile and struck the dovetail of the 777. The employee was staged while being loaded by a 992 when the incident occurred. As a result of the incident, the employee sustained soft tissue injuries to the neck and lower back.

While installing V-belts on the P-2 pump an employee right middle finger was caught between the V-belt and the sheave of the pump motor. As a result, the right middle finger was fractured and required stitches.

While attempting to turn on a water hose to clean BC14 pulley, an employee was struck in the facial area by a hose coupling that had a broken top clamp, resulting in five stitches.

Employee was changing out an acetylene bottle on EE's truck and while EE was trying to carry the bottle, it slipped out of EE's hands and landed on EE's left foot resulting in an injury. Employee was given restriction and instructed to wear a foot brace.

Employee was standing on a ladder fueling haul truck, the hose fell out of the fuel line and landed on the ground while fuel was still pouring out. While in the process of stepping off the ladder the employee stepped on the fuel hose causing them to trip and fall to the ground resulting in a sprain to left ankle.

While dumping a load employee's haul truck bed came into contact with a high voltage powerline. Employee remained in the cab until it was deemed safe to exit and suffered only minor injuries.

While exiting a haul truck, climbing down the access ladder, the employee stepped on a bolt that was on the ground causing them to roll right ankle resulting in a muscle strain in right hip and groin area.

Employee was walking towards the office to clock in and stepped into a pothole, rolled their right ankle resulting in a hairline fracture. It was dark, and the employee did not see the pothole.

Employee washed loader and drove to the fuel island. Put fuel and grease into the loader then proceeded to drive to the ready line. As they were climbing down the ladder, foot slipped on the rung. Leg went through the gap on the bottom step and they hit their shin. The shin laceration was repaired using 6 stitches and employee was placed on restrictions.

On 2/15/2019, an EE sustained a dislocation to the left index finger while attempting to install a timing belt on a vibratory shaker screen. EE was placed on work restrictions and prescribed an immobilization splint to wear on the left index finger.

EE was stepping off the bed of utility truck to retrieve hardware for cutting edge for Cat 992 FEL. EE took a step to EE's rear left and tripped on a bucket tooth the EE had put on the ground, causing EE to fall to EE's rear left, fracturing EE's left wrist.

Loader Operator was loading haul trucks in the pit when a boulder rolled down the muck pile. EE caught the boulder in the bucket but the loader tipped forward. The boulder rolled out and the loader dropped back down and bounced causing lower back injuries to the Operator.

The employee was exiting bulk truck when right foot slipped off the step causing a strain to right knee.

Employee was climbing down front end loader ladder. When closing the cab door foot slipped and ee started to fall. EE left hand was caught in the railing, spraining it.

EE was accessing freshwater pond catwalk to access an issue that allowed pond to overfill. Using a pallet to cross standing water, ee's foot slipped. Putting hand down to catch self ee jammed wrist, fracturing it.

While lifting tail pulley guard up with hoist to access bearing, the guard became stuck on pulley shaft. Employee tried to free by hand. When it came loose it pinched left pinky finger against the shaft, requiring stitches.

Electrician was pulling tool bag from the bed of the bucket truck. As employee turned to walk away, the bag slid off the bed and jerked employee's left wrist down, chipping a small bone fragment from a wrist bone.

Employee was installing apron feeder flights. As it was being set into place it shifted. The employee was guiding it with employee's left hand, which was cut by the edge of the flight when it shifted.

The employee was assisting the Lead Man with the workplace exam and stepped through a doorway from the MCC to the mixing room for thickening tank flocculent. There was a wet spot on the floor from a roof leak caused by a storm. Employee slipped and sprained left ankle.

Employee was standing idler up under belt with a pry bar. The bar slipped and the employee struck hand on the plant support structure.

The employee was removing bolts on a screen spring with a torch when a piece of debris got behind EE's glasses and into EE's eye.

Exiting loader, boot stuck on last ladder rung, employee lost balance, held on to grab bars. Force jerked right shoulder, spraining it.

After setting a new transformer on its platform there was a 3'x2.5' hole exposed in the metal grate due to repositioning of transformer. An apprentice electrician unhooked his fall harness in order to step around the transformer to remove the hoisting straps. He stepped into the hole in the grating & fell about 10' to the ground. He said his wrist was sore & was taken to a clinic

The miner was working on the removal of the Primary Crusher South Side Sheave. He needed to remove one of the two bolts on the back side using a striking wrench and a 4lb hammer. He put the wrench on the nut using his left hand and started swinging the 4lb hammer from the bottom up. He hit the wrench a few times before it impacted his left hand index finger.

The employee accessed the battery of a man-lift to remove the battery cables after jump starting it. Climbing down his left foot became stuck in the tire lugs. He pushed off and landed awkwardly on his right leg, spraining his right knee.

Maintenance Mechanic I was installing Flexco #2 clips into the BC-38 belt at the Counterweight Pulley at BC-38. The belt moved and the employee was caught between the belt and counterweight pulley resulting in 7 fractured ribs.

Employee was checking screen and bearings were locked up. He had the v belts to the motor off. He took the guard for the timing belt and the belt loose. He began to remove belt. He had right hand on the bottom of the belt and left hand was on the side of the belt. He went to slide the belt off, the counterweights rolled and pinned his hand between the belt and the counterweight.

While loading truck, oversized rocks tumbled into the bucket and caused loader to rock back which jarred operator inside of the cab. He felt pain in his back but declined medical attention at the time. Pain did not reduce and employee asked for medical attention and was treated on 1/15/15.

Employee was walking to get supplies and he slipped on the floor catching himself. His rear end hit the floor. He caught himself with his arm.

Employee was using a utility knife to cut skirting, when the knife slipped and employee cut his little finger on his right hand.

Employee was shoveling the middle deck next to shaker #3. Employee then walked down the staircase towards the catwalk by BC-18 and he slipped on the first step, rolling his left ankle. Employee did not feel his accident was bad enough to go to the doctor at this time. Employee decided to go to the doctor on June 19, 2013; where the doctor put him on restricted duty.

EE was servicing the #0827, 777F Haul Truck. While climbing up the left side of the Haul Truck his left knee struck the bumper of the haul truck and overextended his knee causing a sprain.

EE was helping a scale company remove weights from a scale. The weights were 4" round 52" long and 185 lbs. While moving the second weight, the weight dropped onto the belt and smashed EE's middle finger, right hand between the conveyor structure and one side of the weight. EE's finger was crushed and sustained a fracture open to the tip of the finger.

Employee felt his knee pop while walking up to the MCC at the scrubber. When EE returned to his truck and sat down he felt his knee pop again but louder and accompanied by sharp pain. The pain did not recede so he drove up to the office. His knee was tender and when he tried to leave the office he could not walk due to the pain. He was then taken to the clinic.

Employee was hauling load of rocks when rock fell out of bucket on machine and caused him to run over a 24" rock which caused the loader tire to blow out. In trying to recover, he lowered the bucket to ground which caused machine to go unstable and violently shake. Employee's body was jerked around. He exited machine and collapsed on ground.

Employee was cleaning out the wobbler when a 12" rock rolled down a 6' slope and smashed his finger against a 3/8" plate on a lifting basket. Employee sustained a fracture on his left ring finger, 2 cm laceration to the left ring finger, Contusion to left middle finger & 1 cm laceration to the left middle finger & sutures.

EE was fueling his CAT 980 loader. EE was stepping down to get off of his loader when he missed the last step and fell to the ground.

There was a bin on the ground. EE placed another bin on top of it so he could measure and cut it. The top bin slipped and employees middle finger on his right hand sustained an open fracture and avulsion of the nail.

Employee was cutting metal with a torch. A piece of hot metal fell and burned through his pants and fell into his boot.

Employee was stepping off of her haul unit and pulled a muscle in her left knee.

Employee was riding in the back of a pickup truck when he fell onto the bed of the truck bruising his back.

Employee suffered a probable stroke related to high blood pressure and diabetes, and fell to the ground.

Employee was at screening tower to collect a material sample. He stepped on a wet area and fell.

Employee was washing decking. When he reached up to pull more hose, he tripped over some bars on the ground and fell.

The employee was inside a screen. He reached for a channel and over-reached and felt a pull in his elbow.

Employee was tightening bolts on the primary crusher with a torque wrench. The wrench slipped and caught his middle finger on his left hand in between the multiplier handle and the hammer. The employee sustained a fracture to the tip of his middle finger and also received 4 stitches.

Employee was welding at the primary crusher. While breaking flux off the weld, he felt some of the flux get in his eye.

Employee was setting up a 3 ft ladder beside a table so he could reach a longer ladder. As he was climbing to get on top of the table, he lost his footing on the 2nd step. He hit his knee on the ladder edge and fell to the ground. The edge of the ladder caught his lower leg causing a laceration.

Employee was installing a shaker lip on a screen at the screening tower. The lip got hung and the employee tried to free it up by pushing on it. As it freed up he felt the sharp corner of the metal in the center of his hand.

Employee was helping another employee attach a discharge lip between a shaker and a crusher. When one employee pushed the lip, one side slid in but the other did not. When they tried to push the other side, it slid too far, cutting the employee on the left hand.

Employee was unhooking an air line on a railroad car. When the coupling unhooked it released the air and blew dust under his safety glasses into his eye.

Employee was helping another welder, received welders flash to both eyes.

Employee was washing haul trucks when he began having heat cramps.

Employee was coming down off a stockpile of loose material. He slipped on the loose material and twisted his left knee.

Employee was loading shot rock in the quarry when a large boulder fell into his bucket and jostled him.

A customer truck driver was injured at the mine when he fell from his truck bed while tarping his load. The driver disobeyed Hanson site specific training by exiting his truck and climbing in to his bed. The driver lost his balance when a tarping hook snagged his pant leg which caused him to fall. The driver suffered a fractured right wrist.

Employee was lifting the lid to the crusher when he felt something pull in the back of his left leg.

Employee was helping change a broken screen. Screen was in the air and slipped, falling on employees left hand above his thumb.

Employee was tightening bolts at the scrubber. He was using a hammer to strike a wrench. He hit his finger with the hammer against the wrench and cut his finger.

Employee was loosening a bolt with a 9/16" wrench. The wrench slipped and his hand hit the cover plate.

Employee was using a pipe to adjust a scrubber. The pipe slipped and the employee hit his left hand against the wall and dislodged his thumbnail. He had a laceration under the nail and also a small bone was broken.

EE WAS HELPING TO TIGHTEN A SCREEN WHEN HE SLIPPED & FELL OFF THAT SCREEN. SUSTAINED A LUMBAR STRAIN.

Employee was in the primary crusher welding on the hammers with a hand welder. The fan they use to blow the smoke away from where they were welding blew a welding spark up under his helmet into his eye.

Employee was climbing up a ladder to set the brakes on a railcar. The handles were wet from rain and the employee slipped off to ground. Did not fall, just slipped back to the ground.

Ramond Dominguez was trying to slip the bar from one end of the conveyor box to the other end, part of the bar fell inside the box. Ramon was holding the bar outside of the box as part of the bar fell down the other end went up and he had his hand on the shaft, which caught his hand between the speed reducer thus causing an injury resulting in a crushed thumb on the right hand.

Employee was working on the lagging on BC30 and twisted the wrong way.

EMPLOYEE WAS MOVING THE FRONT LATERAL CABLE WITH A FORKLIFT WHEN HE GOT OUT OF THE FORKLIFT THE CABLE BROKE & HIT HIM IN THE ANKLE. SAID IT ONLY FELT BRUISED & HE HAD FULL RANGE OF MOTION. THE NEXT DAY HIS ANKLE WAS SWOLLEN, THE DR. WHO LOOKED AT IT CLASSIFIED IT AS A CONTUSION.

EMPLOYEE WAS LETTING CABLE OUT FROM THE CORNER CABLE WINCH. THE BRAKE FAILED CAUSING THE WINCH HANDLE TO SPIN OUT OF THE EE'S HAND. WHEN THE HANDLE SPUN IT HIT THE EE'S RIGHT HAND. THE HANDLE CUT HIS FINGERS & CRACKED/BROKE THE BONE IN HIS RING FINGER. EE WAS RELEASING UNDER TENSION.

Ee was working on a ladder. He reached for his meter when he and the ladder both fell 8' to the ground. He hit his right shoulder on a pipe and then landed on his left shoulder.

EE BLOCKED A SHAFT TO PUT A STRAP AROUND IT, AND IT ROLLED OFF ONTO HIS RIGHT FOOT.

OPERATOR WAS ATTEMPTING TO DISCONNECT TRAVERSE CABLE FROM THE DREDGE. THE CABLE WAS STILL UNDER TENSION AND THE OPERATOR WAS IN FRONT OF THE CABLE.WHEN HE LOOSENED THE CLAMP THE CABLE SLIPPED FORWARD STRIKING HIM.

EE STATES HE WAS CLOSING THE HATCHES ON THE BULKTRUCK WHICH HAD NO LOWER LADDER. HE ATTEMPTED TOGET OFF TRUCK & SLIPPED & FELL & CUT HIS UPPER ARM ON A BRACKET.

EMPLOYEE WAS ROLLING UP WELDING CABLES. HE WAS NOT WATCHING WHERE HE WAS STEPPING AND STEPPED DOWN BETWEEN 2 CONCRETE BLOCKS. TWISTED RIGHT KNEE.

Operator making adjustment to winch. Removed safety pin while tension was still applied to cable. Brake did not hold due to tension, winch handle spun hitting operator in the wrist & hand

Operator was attempting to release tension from a hand winch he pulled the safety pin without securing the winch handle, the brake was defective so it spun the handle, which striked the operat ors hand ors hand

EE WAS WALKING TO CONTROL HOUSE AT BC9. AS HE STEPPED ON THE PLATFORM HE STEPPED ON SOME ICE & HE FELL.

EE WAS CHECKING A SOLENOID ON A WELDING MACHINE WHEN HIS THUMB GOT CAUGHT IN THE FAN.

EMPLOYEE STATES: WHILE CONDUCTING A SHOT SERVICE, LOADING HOLES MANUALLY WITH LOADING POLE. THE POLE CAME DOWN FAST THROUGH HANDS AND JERKED NECK AND BACK.

EMPLOYEE WAS PICKING UP A METAL PLATE TO INSTALL ON A FEEDER, HE SLIPPED AND DROPPED THE PLATE ON HIS LEFT FOOT. RESULTED IN TOE CONTUSION. RETURNED TO WORK WITH RESTRICTIONS.

WHILE INSTALLING A VERTICAL SHAFT THE CABLE SLIPPED PINNING EE'S HAND BETWEEN SHAFT & ROATING GEAR.

EE WAS REPLACING BELTS ON A PUMP MOTOR. HIS FINGER GOT CAUGHT BETWEEN THE MOTORS & THE BELTS.

EE STATES: LOADING A SHOT AND TRYING TO RETRIEVEDETONATORS. A GUST OF WIND BLEW THE TOP OF THE ALUMINUM BOX CLOSED ON TOP OF MY HEAD AND KNOCKED ME OFF THE BUMPER OF THE PICKUP TRUCK.

EE WAS USING A RATCHET WRENCH TO AEDJUST A TRUNNION, WRENCH SLIPPED & STRUCK EE IN NECK BELOW JAW.

2 EE WERE STRIKING A PUNCH WITH A HAMMER TO REMOVE AN OLD BOLT. SMALL PIECE OF METAL CAME OFF THE BOLT INTO THE EE EYE.

EMPLOYEE WAS UNLOADING SHEET METAL OFF OF A TRAILER. THE METAL SLIPPED FROM HIS GRIP AND CUT 4 FINGERS ON LEFT HAND.

EE WAS GRINDING A PIECE OF STEEL. GOT A STEEL SHAVING IN HIS LEFT EYE.

EE WAS STANDING AT THE TOP OF THE STAIRS AND FELL DOWN 5-6 STEPS. INJURY DID NOT APPEAR SERIOUS AT FIRST, BUT HAS CONTINUED TO HURT. HE MAY REQUIRE SURGERY.

EE WAS CLEANING AT THE LODGE--WAS STUNG BY 2 RED WASPS ON RIGHT ARM ELBOW AREA.

EE WAS HELPING ANOTHER EE TIGHTEN BOLTS ON A SPROCKET THE WRENCH SLIPPED OFF THE NUT STRIKINGLEFT HAND

HIT RIGHT ELBOW ON INSIDE DOOR OF DOZER.

EMPLOYEE WAS CLEANING WITH A SHOVEL, TURNED AROUND TO DUMP SHOVEL AND FELT PAIN IN LOWER BACK.

EMPLOYEE WAS UNCHOKING THE CRUSHER WHEN THE INSPECTION DOOR ON THE CRUSHER FELL ON HIS RIGHT FOOT, CAUSING A FRACTURE.

EE WAS SWITCHING CARS STEPPED DOWN OFF CAR & TWISTED KNEE

EE WAS CLIMBING DOWN OFF TRUCK WHEN HE FELT A PULL IN HIS BACK.

EMPLOYEE WAS SQUATTING TO REMOVE COLTER KEY. WHEN HE STOOD UP HE CAUGHT HIS LEFT FOREARM ON A SHARP METAL BURR ON THE COUPLER KNUCKLE LACERATING LEFT FOREARM.

EE WAS CHECKING MATERIAL IN RAIL CARS. DISMOUNTED CAR INCORRECTLY. FOOT BECAME LODGED AND HE CAME DOWN ON ONE LEG. FRACTURED RT KNEE WHERE FIBULA JOINS THE KNEE.

FATAL HEART ATTACK

CRUSHER WAS STOPPED UP WITH A VERYLARGE ROCK. EE WAS USING A SLEDGE HAMMER TO LOOSEN BLOCKAGE. CCHIP OF THE BOULDER FLEW UP AND HIT HIM ABOVE LEFT EYE.

EMPLOYEE WAS COMING DOWN STAIRS LOST HIS BALANCEAND USED HIS HANDS TO BREAK HIS FALL.

EE WAS CLIMBING STAIRS AT NEW LOADOUT WHEN HE FELT A BAD CRAMP IN THE BACK OF HIS RIGHT LEG.

WAS REMOVING BLOCKING ON A UNIT WHEN A METAL B BLOCK USED AS A SPACER HAD HUNG. WHEN IT WAS KNOCKED OUT IT HIT EE ON RIGHT KNEE CAP

EE WAS WORKING ON A BELT WHEN ANOTHER EE STARTED THAT BELT BY ACCIDENT. EE FELL OFF CONVEYOR INTO THE HOPPER.

EMPLLOYEE WAS CUTTING CARDBOARD WHEN HIS KNIFE CLOSED AND CUT THE BACK OF HIS MIDDLE FINGER ON RIGHT HAND. HAD 3 STITCHES.

EE WAS TAKING A BEARING OFF THE MESABI SHAKER AT THE PORTABLE PLANT. HE SLIPPED AND CAUGHT HIS FOOT ON A V BELT THAT WAS HANGING OFF SIDE OF SHAKER. HE FEEL 7FT TO THE GROUND

EE WAS IN HIS HAUL UNTIL WITH HIS FOOT AGAINST THE DOOR. TO TIE HIS SHOELACES THE DOOR CAME OPEN AND THE FELL OUT ONTO THE GROUND.

WHILE USING A DRILL TO DRILL A HOLE, DRILL SEIZED UP AND HE PULLED HIS RIGHT SHOULDER.

WHILE INSTALLING TIRES IN A DUMP HOPPER AND BEING OFF WITH CABLE, A SHARP PIECE OF CABLE OUT HIS FINGER.

EE WAS USING A HAMMER TO PULL OUT A BEARING. BEARING BROKE & PIECE OF METAL HIT & CUT LEFT FOREARM.

WHILE INSTALLING A CHAIN ON A SPROCKET, THE CHAIN SLIPPED OUT OF HIS HAND & CUT THE TIP OF HIS RIGHT INDEX FINGER.

WHILE REPAIRING AXLW ON LOCOMOTIVE, BLOCK WAS PUSHED BACK & HIT HIM ON LITTLE FINGER OF RIGHT HAND.

EE WAS USING RESTROOM AT PORTALET WHEN SHE FELT SOMETHING BITE HER IN TALBONE AREA.

EMP WAS WORKING ON BRAKES UNDER A HAUL UNIT WHENHE FELT SOMETHING IN HIS LEFT EYE.

EMPLOYEE WAS CHECKING ELVATOR USING CONTROL BOX. FUSE BOX SHORTED OTU AND PANEL HIT EMPLOYEE INF ACE. BROKE NOSE. CONTROLS BURNED HANDS.

WHILE PICKING UP A BUSHING, IT SLIPPED. HE TRIED TO KEEP IT FROM FALLING, HIT BUSHING WITH THUMB.

EE WAS MOVING FORMS ON GROUND WHEN HAD SLIPPED AND ELBOW LEFT HIK ANOTHER FORM BEHIND HIM.

EE WAS DIPPING TROWELS INTO BUCKET OF WATER AND RUBBING TROWELS TOGETHER TO GET EXCESS CONCRETE OFF. TROWELS WERE STEEL TROWELS AND THE EDGES BECAME SHARP AFTER WEAR. EE WAS NOT WEARING HIS GL OVES DUE TO WORKING IN WATER.

EE WAS USING HOOK TO LOOSEN ROCKS FROMA CHOKE UP HOOH SLIPPED AND CAUSED EE TO SPRAIN BACK.

EE WAS STEAM CLEANING UNIT. WHEN HE CRAWLED UP INTO BACK OF TRUCK HE LAYED WAND DOWN. WHEN HE PICKED IT UP IT SLIPPED, HITTING HIM ON LEFT ANKLE.

EE WAS OPERATING EUCLID WHEN ANOTHER EE IN A EUCLID CAME AROUND THE CORNER & HIT HIM.

EE WAS COMING AROUND A CORNER IN HIS EUCLID, WHEN HE HIT ANOTHER EUCLID COMING THE OTHER WAY.

EE WAS TAKING BOBCAT TO PORTABLE PLANT WHEN HE WAS HIT FROM THE RIGHT SIDE BY A HAUL UNIT. HAUL UNIT OPERATE SAID HE JUST DID NOT SEE HIM.

EE WAS SETTING BRAKES ON RAILCAR. BRAKE WAS STIFF & WHEN IT FREED ITSELF, HE FELL BACKWARD WHILE STILL HOLDING ONTO THE WHEEL.

EE WAS LIFTING COUNTERWEIGHT CAME DOWN ON RIGHT FOOT.

WHILE TRAVELING DOWN HAUL ROAD, EE HIT A POT HOLE AND STRAINED HIS BACK.

EMPLOYEE WAS HELPING INSTALL BELTING. THE WIND WAS BLOWING HARD AND HE FELT SOMETHING IN HIS EYE.

EMPLOYEE WAS MOVING A RAILROAD TIE THAT WAS TOO HEAVY. FELT A PULL IN LOWER BACK.

WHILE INSTALLING A PLATE AT THE IMPACT CRUSHER, LIFTED PLATE BY HIMSELF. FELT A PULL IN STOMACH.

EE WAS HELPING INSTALL V-BELTS. WHEN BELT ROLLED BACKWARD, HIS HAND GOT CAUGHT BETWEEN THE BELTS AND THE PULLEY.

EMPLOYEE MADE A HOLE IN GUARD TO GREASE PULLEY. BAR HIT PULLEY & CAME BACK & HIT HIM IN UPPER FACE.

EMPLOYE EWAS CLOSING & LOCKIGN DOORS ON OPEN TOP HOPPERS. ORE DOOR WAS DIFFICULT. HE STRAIEND HIS LOWER BACKAS HE WAS PULLING UP ON THE BAR.

EMPLOYEE WAS CLEARING RAIL CAR. WHILE SWINGING SLEDGE HAMMER, FELT PAIN IN UPPER RIGHT ARM.

EMPLOYEE WAS ATTEMPTING TO MOVE A GREASE DRUM WHEN HE FELT A PAIN IN HIS LOWER RIGHT SIDE.

EE WAS REMOVING SPINDLE ASSEMBLY WITH THE AID OF A PRYBAR WHEN IT FELL, CAUSING BAR TO KICK BACK STRIKING, EE ON LEFT JAW.

EMPLOYEE WAS USING A BAR TO PUSH A ROCK OUT OF A CONVEYOR FRAME, THEN USED HIS LEFT HAND TO PICK THE ROCK. GOT HIS LEFT HAND CAUGHT BETWEEN CONVEYOR IDLER AND BREAKING THE LITTLE FINGE RAND A BOND IN HIS HAND.

EE WAS CLEANING DEBRI BENT OVER USING A PICH. WHEN HE FINISHED THIS JOB HE INJOINED FOREMAN HE HAD A PAIN IN HIS LOWER RIGHT BACK.

AS HE WAS PUTTING IN THE MASTER SWITCH HE GOT HIS LEFT RING FINGER CAUGHT BETWEEN THE LEVER & THE SWITCH BOX

REPORTED TO PERSON THAT HE WAS HAVING PLAIN IN HIS RIGHT LOWER HIP POSSIBLY FROM LIFTING SOME PARTS THE DAY BEFORE 11-9-93.

EE WAS NOT PAYING ATTENTION TO WHAT HE WAS DOING & DROVE OVER AN EMBANKMENT.

EE WAS PUTTING A PROTECTIVE COVER BACK ON THE MAINTAINER #2371. THE BOTTOM SLIPPED DOWN AS THE PREP POPPED OFF HITTING HIM IN THE EYE WITH A SHARP EDGE.

EE WAS CHECKING OIL IN UNIT FELL OFF UNIT ON TO TIRE.

EMPLOYEE WAS USING A COME-ALONG. IT SLIPPED CATCHING & CUTTING HIS LEFT HAND BETWEEN THE THUMB AND FOREFINGER.

EMPL HELPING PACK STEEL PLATE. LOST FOOTING AND FELL FRACTURING LEFT HAND RING FINGER.

EE WAS CLIMBING BACK ON EUCLID SLIPPED ON A DAMP STEP AND FELL TO GROUND.

EMPLOYEE WAS CLEANING ROLELRS ON CONVEYOR BELT WITHA HOE. HE GOT THE HOE AND HIS LEFT HAND CAUGHT BETWEEN THE ROLLERS AND BELT, CUTTING LEFT THUMB OFF AT FIRST JOINT.

EE WAS USING A STEEL BAR TO INSTALL FEEDBOX ON CRUSHER BAR SLIPPED EE FELL BACK AGAINST ANOTHER FEEDBOX.

54736YEE WAS ATTEMPTING TO ALIGN THE RADIATOR ON UNIT #0113.THE BAR THAT HE WAS USING SLIPPED OFF AND HE HIT HIMSELF IN THE LEFT GROIN AREA,CAUSING A SWELLING IN THAT AREA.

54736YEE WAS USING A HAMMER TO CLEAN A PLACE TO WELD UNDER THE BIG IMPACT CRUSHER WHEN HE GOT SOME DIRT IN HIS EYES.

EMPLOYEE WAS GETTING OFF THE SERVICE TRUCK. WHEN HE STEPPED OFF THE LADDER, HE STEPPED ON A ROCK, CAUSING HIM TO TWIST HIS LEFT ANKLE.

EMPLOYEE WAS CLEANING OUT THE BASE HOPPER. HE LOWERED HIMSELF 3 FEET INTO THE HOPPER, WEARING A SAFETY ROPE. HE PULLED A MUSCLE IN HIS SIDE THE DOCTOR LATER TOLD HIM HE HAD IRITATED SOME SCAR TISSUE THAT HE HAD FROM AN APPENDICITIS OPERATION SEVERAL YEARS AGO.

EMPLOYEE WAS CLEANING BASE HOPPER WHEN HE SLIPPED AND SLID DOWN THE SIDE OF THE HOPPER TO THE BOTTOM (ABOUT 6') OVER THE WEEKEND HE BEGAN TO EXPERIENCE PAINS IN HIS BACK AND ASKED TO SEE A DR. ACCIDENT WAS NOT REPORTED UNTIL MONDAY, APRIL 6TH.

EMPLOYEE PUT DIESEL IN HIS TRUCK AND CLIMBED BACK UP THE LADDER. HIS FOOT SLIPPED ON THE CATWALK AND HE FELL, SCRAPING HIS LEFT ELBOW AND RIGHT KNEE.

EMPLOYEE WAS CLEANING AT THE BASE PLANT & GOT THE SHOVEL & HIS TWO FINGERS ON HIS LEFT HAND CAUG HT BETWEEN THE CONVEYOR IDLER & A METAL SKIRT

EMPLOYEE WAS OPERATING LOADER, HE DROVE ACROSS RAILROAD TRACKS TOO FAST AND THE SEAT HIT THE BOTTOM OF THE FRAME, HURTING HIS LOWER BACK.

EMPL REINSTALLING DUST COLLECTOR ON DRILL.AS HE WAS PULLING WITH A BAR,FELT SLIGHT PAIN IN LOWER BACK.

EE TRIED TO CLIMB ABOARD 9 RUNAWAY RAILS CARS ON #3 TRACK. HE COULD NOT GET A GOOD HOLD ON THE CARS, SLIPPED AND FELL, SKINNED HIS LEFT FOREARM AND BROKE HIS WATCH. LATER COMPLAINED OF BACK PA IN.

EMPL WENT INTO #2 TUNNEL TO CHANGE OVER FEEDER.CAME OUT,SLIPPED & FELL ON RIGHT ARM.

EMPLOYEE WAS WORKING ON A CHUTE BY THE 10K CRUSHER HE LOST HIS FOOTING FELL & HIT HIS RT ELBOW ON CHUTE

THIS CASE IS CURRENTLY UNDER INVESTIGATION.T HIS MAN HAS VERY SERIOUS ARTHRITIS WHICH HE CLAIMS IS A RESULT OF THE MANY YEARS HE HAS RUN A BULLDOZER HERE, AMONG SEVERAL OTHER PIECES OF MACHINE RY HE HAS OPERATORED AT ONE TIME OR ANOTHER. MAINLY THE DOZER. HE HAS A DR REPORT THAT STATES HE THINGKS IT MAY BE DUE TO WORK BUT HE WILL NOT SAY FOR SURE. HE HAS NO DATE FOR THE INJURY OR I

EE PICKED UP A COME LONG FROM THE CATWALK AND PUT IN ON THE CONVEYOR BELT AND FELT A PULL TO HIS LEFT SHOULDER.

EMPL WAS TAKING BABBIT BEARING OFF HE HIT IT WITH SLEDGEHAMMER & PIECE OF STEEL IN RIGHT EYE.

EMPLOYEE WAS PATCHING A CONVEYOR BELT WITH AN IMPACT WRENCH AT THE OE PRIMARY PLANT A PIECE OF BELT FLEW OFF & BROKE LOOSE FROM THE SOCKET SENDING A FOREIGN OBJECT INTO THE EMPLOYEES LEFT EYE

EE WAS SITTING ON BELT GREASING CONVEYOR PULLEY WHEN HIS FOREARM TURNED THE BELT ON. HE FELL 10-20 FT ONTO THE STOCKPILE BELOW.

FIRST ACCIDENT WAS WHEN SHE WALKED AROUND THE CORNER IN THE OFFICE & BANGED HER KNEE ON A CHAIR SECOND SHE WAS WALKING OUT THE FRONT DOOR AFTER WORK & FELL DOWN THE STEPS IN FRONT OF THE OFFIC E THESE BOTH CONTRIBUTED TO HER REQUIRING SURGERY AFTER SHE WAS TERMINATED ON 02-13-91

EMPL CLIMBING ON EUCLID FELT RIGHT KNEE TWIST AND IT STARTED SWELLING.EMPL TERMINATED 12-14-90.EMPL SOUGHT MEDICAL ATTENTION AFTER TERMINATED.UNDERWENT SURGERY 2-25-91.

EMPLOYEE WAS WORKING ON THE RAIL. HE WAS HOLDING A BAR TO LIFT RAIL INTO PLACE FOR SPIKING. HE WAS HIT ON THE HAND WITH A SLEDGE HAMMER.

EMPLOYEE WAS CLOSING AND LOCKING DOORS ON RAIL CARS WHEN THE LITTLE FINGER ON HIS RIGHT HAND WAS CAUGHT BETWEEN THE BAR AND SIDE POST. AS THE BAR WAS MOVING IN A COUNTER-CLOCKWISE DIRECTION, H IS LITTLE FINGER BENT OVER BACKWARDS.

CASE IS CURRENTLY UNDER INVESTIGATION.EMPL STATED TO MGR AND OTHERS THAT INJURY WAS AT HOME. WHEN DISCOVERED SURGERY WAS INDICATED,RELATED INJURY TO ACCIDENT ON LOADER.LIP OF BUCKET ON LOADER GOT CAUGHT ON RAIL AND JERKED HIM FORWARD.

EMPLOYEE SLIPPED ON SOME OIL THAT HAD SPILLED ON THE BACK OF HIS FUEL TRUCK,FALLING 3-4 FT TO THE GROUND.

ON ORIGINAL REPORT,6-5-89,EMPL STATED :DID A SUDDEN TURN AND FELT SOMETHING POP TO LEFT BUTTOCK.REOCCURRANCE OF THIS INJURY,SOME BLOODWORK & PATHOLOGY.

EMPLOYEE WAS DESCENDING THE LADDER ON A GIHX SERIES RAIL CAR. AS HE STEPPED ON THE PLATFORM, HE TURNED HIS LEFT ANKLE ON A WORN BRAKE SHOE THAT HAD BEEN LEFT LYING THERE.

EMPLOYEE COMPLAINED OF NUMBNESS TO HANDS AND ARMS.

EMPL STATES HE PULLED LEFT SHOULDER WHILE SHOVELING AT PRIMARY CRUSHER.

EMPLOYEE GOT LEFT HAND CAUGHT BETWEEN CONVEYOR ROLLER AND STEEL BAR THAT WAS BEING USED FOR REPAIR. EMPLOYEE RECEIVED LACERATION FROM THUMB TO MID PALM OF HAND.

REMOVING ROCK BY HAND FROM BETWEEN THE BELT ROLLER AT THE SHUTTLE BELT WHEN HE GOT HIS HAND CAUGHT

CLEARING BRUSH ON BACK SECTION OF QUARRY, STUNG BY QUITE FEW TIMES AROUND FACE AND NECK BY BEES.

EMPL USING IRON BAR TO HOLD CONVEYOR BELTING IN PLACE WHILE FASTENEING.BAR SLIPPED HITTING EMPL BEHIND RIGHT EAR.

EMPLOYEE WAS TURNING ON WATER PUMP TO OFFICE HE STEPPED ON A SMALL PILE OF SAND & IT GAVE WAY CAUSING HIM TO TWIST HIS RIGHT ANKLE

EMPLOYEE WAS CUTTING SOME SKIRTING WITH A UTILITY KNIFE,THE KNIFE SLIPPED AND CUT A 1" INCISION IN HER RIGHT LEG ABOVE HER KNEE.

EMPLOYEE STATED HE DID A SUDDEN TURN AND FELT SOMETHING POP TO LEFT BUTTOCK.

EMPLOYEE WAS LOADING A RAILROAD HOPPER CAR WITH MATERIAL. BRAKES LOCKED ON LOADER CAUSING IT TO ROCK BACK AND FORTH, AND EMPLOYEE WAS JERKED WITH IT. COMPLAINED OF ACHE IN LOWER BACK.

EMPLOYEE STATES HE WAS HELPING THE WELDER FROM 9 AM TO 5:00 PM WHILE AT RINSING PLANT WELDING GUARDRAIL AT THE SANDGUTTER (CHUTE). COMPLAINED OF BURNING SENSATION TO FACE.

EMPL USING HOOK TO DISLODGE ROCKS FROM 10K CRUSHER LARGE ROCK HIT HOOK,RIGHT HAND SMALL FINGER HYPEREXTENDED.

EMPL WAS HELPING UNLOAD A CONVEYOR FROM A TRUCK WHEN THE CHAIN AROUND THE SATCH BLOCK SLIPPED CAUSING THE CONVEYOR TO SHIFT WHEN THIS HAPPENED THE EMPL JUMPED FROM THE TRUCK TO AVOID BEING HI T BY THE CONVEYOR WHEN HE HIT THE GOUND HE TWISTED HISANKLE AND LANDED ON HIS SIDE AND WRIST CAUSING THEM TO BE BRUISED

EMPLOYEE WAS INSIDE WELDING SHOP STANDING ON HORSE TIEING CHAINS TO SHAKER. IT STARTED COMING DOWN AND HIS LEFT FOOT GOT CAUGHT BY BOOT AT CORNER OF HORSE. HE FELL, PUTTING WEIGHT ON LEFT FO OT AS IT TWISTED.

EMPLOYEE WAS HELPING ELECTRICAL CREW INSTALL TUBE AT SAND CLASSIFIER HE FELT SOMETHING IN HIS EYE HE CLEANED OUT HIS EYE BUT IT WAS STILL HURTING & HE WENT TO THE DR.

EMPLOYEE WAS REMOVING LARGE ROCK FROM CRUSHER AND HAD SUDDEN ONSET OF LOWER BACK PAIN. HE WAS USING A STEEL HOOK TO DISLODGE THE ROCK.

EMPLOYEE WAS USING A VICE TO HOLD A PIECE OF PIPE HE DROPPED THE PIPE & HIT HIS RT.ANKLE IN THE FOOT AREA

EMPLOYEE WAS TRYING TO REMOVE TIRE FROM A EUCLID USING A METAL BAR TO PRY BAR SLIPPED AND HIT EMPLOYEE ON RT UPPER THIGH

EMPL AT RAPID SCREEN TIGHTENING BOLTS WITH WRENCH.USING ANOTHER PIPE FOR LEVERAGE.WRENCH SLIPPED MAKING PIPE AIRBORNE HITTING I.NAME ABOVE RIGHT EYEBROW.

EMPLOYEE WAS DISLODGING ROCK FROM CHUTE AT O.E. PLANT. LEFT INDEX FINGER GOT CARUGHT BETWEEN LARGE ROCK AND STEEL ROD. COMPLAINED OF PAIN AND UNABLE TO FLEX FINGER, SWELLING NOTED.

EMPLOYEE WENT INTO SWITCH HOUSE TO START UP PLANT.HE CAME OUT AND SAID HE HAD PUSHED HIS LEFT AHND THROUGH THE WINDOW AND CUT HIS WRIST.

EMPLOYEE REACHED UNDER A DESK IN THE SALES DEPT TO PICK UP SOME PAPER AND HIS LEFT FOOT SLIPPED OUT FROM UNDER HIM CAUSING HIM TO FALL ON HIS RIGHT KNEE. HE TWISTED HIS KNEE, CAUSING IT TO SW ELL UP AND KEEP HIM FROM WALKING WELL.
