EE was working installing a liner and while positioning EE slipped and fell. There was excess water and mud in the area. EE fell and dropped the liner which caused EE's fracture and laceration to EE's left index finger.
Employees were putting a guard back on after replacing a bad idler when the guard slipped and hit one employee in the upper lip requiring six sutures.

EE reports hydraulic ladder pinched right leg between the handrail and the folding ladder. EE was assisting a third-party mechanic while the mechanic was troubleshooting the hydraulic ladder. Contacted WorkCare and after a few days of self-care, the EE was scheduled for a CV. EE was diagnosed with a torn ligament in right calf area.

An employee was conducting a pre-use equipment checklist when the employee slipped and fell from a water truck. The employee received sutures to the back of the head.
During the process of fueling the locomotive, the Locomotive Engineer received diesel splash back in the eyes from nozzle located at locomotive fuel island.

An employee was operating a CAT 777 Haul Unit in the L2 pit, when a boulder rolled out of the muck pile and struck the dovetail of the 777. The employee was staged while being loaded by a 992 when the incident occurred. As a result of the incident, the employee sustained soft tissue injuries to the neck and lower back.
While installing V-belts on the P-2 pump an employee right middle finger was caught between the V-belt and the sheave of the pump motor. As a result, the right middle finger was fractured and required stitches.

While attempting to turn on a water hose to clean BC14 pulley, an employee was struck in the facial area by a hose coupling that had a broken top clamp, resulting in five stitches.
Employee was changing out an acetylene bottle on EE's truck and while EE was trying to carry the bottle, it slipped out of EE's hands and landed on EE's left foot resulting in an injury. Employee was given restriction and instructed to wear a foot brace.
Employee was standing on a ladder fueling haul truck, the hose fell out of the fuel line and landed on the ground while fuel was still pouring out. While in the process of stepping off the ladder the employee stepped on the fuel hose causing them to trip and fall to the ground resulting in a sprain to left ankle.

While dumping a load employee's haul truck bed came into contact with a high voltage powerline. Employee remained in the cab until it was deemed safe to exit and suffered only minor injuries.
While exiting a haul truck, climbing down the access ladder, the employee stepped on a bolt that was on the ground causing them to roll right ankle resulting in a muscle strain in right hip and groin area.
Employee was walking towards the office to clock in and stepped into a pothole, rolled their right ankle resulting in a hairline fracture. It was dark, and the employee did not see the pothole.

Employee washed loader and drove to the fuel island. Put fuel and grease into the loader then proceeded to drive to the ready line. As they were climbing down the ladder, foot slipped on the rung. Leg went through the gap on the bottom step and they hit their shin. The shin laceration was repaired using 6 stitches and employee was placed on restrictions.

On 2/15/2019, an EE sustained a dislocation to the left index finger while attempting to install a timing belt on a vibratory shaker screen. EE was placed on work restrictions and prescribed an immobilization splint to wear on the left index finger.

EE was stepping off the bed of utility truck to retrieve hardware for cutting edge for Cat 992 FEL. EE took a step to EE's rear left and tripped on a bucket tooth the EE had put on the ground, causing EE to fall to EE's rear left, fracturing EE's left wrist.
Loader Operator was loading haul trucks in the pit when a boulder rolled down the muck pile. EE caught the boulder in the bucket but the loader tipped forward. The boulder rolled out and the loader dropped back down and bounced causing lower back injuries to the Operator.
The employee was exiting bulk truck when right foot slipped off the step causing a strain to right knee.
Employee was climbing down front end loader ladder. When closing the cab door foot slipped and ee started to fall. EE left hand was caught in the railing, spraining it.

EE was accessing freshwater pond catwalk to access an issue that allowed pond to overfill. Using a pallet to cross standing water, ee's foot slipped. Putting hand down to catch self ee jammed wrist, fracturing it.
While lifting tail pulley guard up with hoist to access bearing, the guard became stuck on pulley shaft. Employee tried to free by hand. When it came loose it pinched left pinky finger against the shaft, requiring stitches.
Electrician was pulling tool bag from the bed of the bucket truck. As employee turned to walk away, the bag slid off the bed and jerked employee's left wrist down, chipping a small bone fragment from a wrist bone.
Employee was installing apron feeder flights. As it was being set into place it shifted. The employee was guiding it with employee's left hand, which was cut by the edge of the flight when it shifted.
The employee was assisting the Lead Man with the workplace exam and stepped through a doorway from the MCC to the mixing room for thickening tank flocculent. There was a wet spot on the floor from a roof leak caused by a storm. Employee slipped and sprained left ankle.

The employee was removing bolts on a screen spring with a torch when a piece of debris got behind EE's glasses and into EE's eye.

Exiting loader, boot stuck on last ladder rung, employee lost balance, held on to grab bars. Force jerked right shoulder, spraining it.
After setting a new transformer on its platform there was a 3'x2.5' hole exposed in the metal grate due to repositioning of transformer. An apprentice electrician unhooked his fall harness in order to step around the transformer to remove the hoisting straps. He stepped into the hole in the grating & fell about 10' to the ground. He said his wrist was sore & was taken to a clinic
The miner was working on the removal of the Primary Crusher South Side Sheave. He needed to remove one of the two bolts on the back side using a striking wrench and a 4lb hammer. He put the wrench on the nut using his left hand and started swinging the 4lb hammer from the bottom up. He hit the wrench a few times before it impacted his left hand index finger.
The employee accessed the battery of a man-lift to remove the battery cables after jump starting it. Climbing down his left foot became stuck in the tire lugs. He pushed off and landed awkwardly on his right leg, spraining his right knee.
Maintenance Mechanic I was installing Flexco #2 clips into the BC-38 belt at the Counterweight Pulley at BC-38. The belt moved and the employee was caught between the belt and counterweight pulley resulting in 7 fractured ribs.

Employee was checking screen and bearings were locked up. He had the v belts to the motor off. He took the guard for the timing belt and the belt loose. He began to remove belt. He had right hand on the bottom of the belt and left hand was on the side of the belt. He went to slide the belt off, the counterweights rolled and pinned his hand between the belt and the counterweight.
While loading truck, oversized rocks tumbled into the bucket and caused loader to rock back which jarred operator inside of the cab. He felt pain in his back but declined medical attention at the time. Pain did not reduce and employee asked for medical attention and was treated on 1/15/15.

Employee was walking to get supplies and he slipped on the floor catching himself. His rear end hit the floor. He caught himself with his arm.
Employee was using a utility knife to cut skirting, when the knife slipped and employee cut his little finger on his right hand.
Employee was shoveling the middle deck next to shaker #3. Employee then walked down the staircase towards the catwalk by BC-18 and he slipped on the first step, rolling his left ankle. Employee did not feel his accident was bad enough to go to the doctor at this time. Employee decided to go to the doctor on June 19, 2013; where the doctor put him on restricted duty.
EE was servicing the #0827, 777F Haul Truck. While climbing up the left side of the Haul Truck his left knee struck the bumper of the haul truck and overextended his knee causing a sprain.
EE was helping a scale company remove weights from a scale. The weights were 4" round 52" long and 185 lbs. While moving the second weight, the weight dropped onto the belt and smashed EE's middle finger, right hand between the conveyor structure and one side of the weight. EE's finger was crushed and sustained a fracture open to the tip of the finger.
Employee felt his knee pop while walking up to the MCC at the scrubber. When EE returned to his truck and sat down he felt his knee pop again but louder and accompanied by sharp pain. The pain did not recede so he drove up to the office. His knee was tender and when he tried to leave the office he could not walk due to the pain. He was then taken to the clinic.

Employee was hauling load of rocks when rock fell out of bucket on machine and caused him to run over a 24" rock which caused the loader tire to blow out. In trying to recover, he lowered the bucket to ground which caused machine to go unstable and violently shake. Employee's body was jerked around. He exited machine and collapsed on ground.
Employee was cleaning out the wobbler when a 12" rock rolled down a 6' slope and smashed his finger against a 3/8" plate on a lifting basket. Employee sustained a fracture on his left ring finger, 2 cm laceration to the left ring finger, Contusion to left middle finger & 1 cm laceration to the left middle finger & sutures.

EE was fueling his CAT 980 loader. EE was stepping down to get off of his loader when he missed the last step and fell to the ground.
There was a bin on the ground. EE placed another bin on top of it so he could measure and cut it. The top bin slipped and employees middle finger on his right hand sustained an open fracture and avulsion of the nail.

Employee was riding in the back of a pickup truck when he fell onto the bed of the truck bruising his back.
Employee suffered a probable stroke related to high blood pressure and diabetes, and fell to the ground.
Employee was at screening tower to collect a material sample. He stepped on a wet area and fell.
Employee was washing decking. When he reached up to pull more hose, he tripped over some bars on the ground and fell.
The employee was inside a screen. He reached for a channel and over-reached and felt a pull in his elbow.

Employee was tightening bolts on the primary crusher with a torque wrench. The wrench slipped and caught his middle finger on his left hand in between the multiplier handle and the hammer. The employee sustained a fracture to the tip of his middle finger and also received 4 stitches.
Employee was welding at the primary crusher. While breaking flux off the weld, he felt some of the flux get in his eye.
Employee was setting up a 3 ft ladder beside a table so he could reach a longer ladder. As he was climbing to get on top of the table, he lost his footing on the 2nd step. He hit his knee on the ladder edge and fell to the ground. The edge of the ladder caught his lower leg causing a laceration.
Employee was installing a shaker lip on a screen at the screening tower. The lip got hung and the employee tried to free it up by pushing on it. As it freed up he felt the sharp corner of the metal in the center of his hand.
Employee was helping another employee attach a discharge lip between a shaker and a crusher. When one employee pushed the lip, one side slid in but the other did not. When they tried to push the other side, it slid too far, cutting the employee on the left hand.
Employee was unhooking an air line on a railroad car. When the coupling unhooked it released the air and blew dust under his safety glasses into his eye.

Employee was washing haul trucks when he began having heat cramps.
Employee was coming down off a stockpile of loose material. He slipped on the loose material and twisted his left knee.
Employee was loading shot rock in the quarry when a large boulder fell into his bucket and jostled him.
A customer truck driver was injured at the mine when he fell from his truck bed while tarping his load. The driver disobeyed Hanson site specific training by exiting his truck and climbing in to his bed. The driver lost his balance when a tarping hook snagged his pant leg which caused him to fall. The driver suffered a fractured right wrist.
Employee was lifting the lid to the crusher when he felt something pull in the back of his left leg.
Employee was helping change a broken screen. Screen was in the air and slipped, falling on employees left hand above his thumb.

Employee was tightening bolts at the scrubber. He was using a hammer to strike a wrench. He hit his finger with the hammer against the wrench and cut his finger.
Employee was loosening a bolt with a 9/16" wrench. The wrench slipped and his hand hit the cover plate.
Employee was using a pipe to adjust a scrubber. The pipe slipped and the employee hit his left hand against the wall and dislodged his thumbnail. He had a laceration under the nail and also a small bone was broken.
EE WAS HELPING TO TIGHTEN A SCREEN WHEN HE SLIPPED & FELL OFF THAT SCREEN. SUSTAINED A LUMBAR STRAIN.

Employee was in the primary crusher welding on the hammers with a hand welder. The fan they use to blow the smoke away from where they were welding blew a welding spark up under his helmet into his eye.
Employee was climbing up a ladder to set the brakes on a railcar. The handles were wet from rain and the employee slipped off to ground. Did not fall, just slipped back to the ground.
Ramond Dominguez was trying to slip the bar from one end of the conveyor box to the other end, part of the bar fell inside the box. Ramon was holding the bar outside of the box as part of the bar fell down the other end went up and he had his hand on the shaft, which caught his hand between the speed reducer thus causing an injury resulting in a crushed thumb on the right hand.
Employee was working on the lagging on BC30 and twisted the wrong way.
EMPLOYEE WAS MOVING THE FRONT LATERAL CABLE WITH A FORKLIFT WHEN HE GOT OUT OF THE FORKLIFT THE CABLE BROKE & HIT HIM IN THE ANKLE. SAID IT ONLY FELT BRUISED & HE HAD FULL RANGE OF MOTION. THE NEXT DAY HIS ANKLE WAS SWOLLEN, THE DR. WHO LOOKED AT IT CLASSIFIED IT AS A CONTUSION.
EMPLOYEE WAS LETTING CABLE OUT FROM THE CORNER CABLE WINCH. THE BRAKE FAILED CAUSING THE WINCH HANDLE TO SPIN OUT OF THE EE'S HAND. WHEN THE HANDLE SPUN IT HIT THE EE'S RIGHT HAND. THE HANDLE CUT HIS FINGERS & CRACKED/BROKE THE BONE IN HIS RING FINGER. EE WAS RELEASING UNDER TENSION.
Ee was working on a ladder. He reached for his meter when he and the ladder both fell 8' to the ground. He hit his right shoulder on a pipe and then landed on his left shoulder.

EE BLOCKED A SHAFT TO PUT A STRAP AROUND IT, AND IT ROLLED OFF ONTO HIS RIGHT FOOT.
OPERATOR WAS ATTEMPTING TO DISCONNECT TRAVERSE CABLE FROM THE DREDGE. THE CABLE WAS STILL UNDER TENSION AND THE OPERATOR WAS IN FRONT OF THE CABLE.WHEN HE LOOSENED THE CLAMP THE CABLE SLIPPED FORWARD STRIKING HIM.
EE STATES HE WAS CLOSING THE HATCHES ON THE BULKTRUCK WHICH HAD NO LOWER LADDER. HE ATTEMPTED TOGET OFF TRUCK & SLIPPED & FELL & CUT HIS UPPER ARM ON A BRACKET.
EMPLOYEE WAS ROLLING UP WELDING CABLES. HE WAS NOT WATCHING WHERE HE WAS STEPPING AND STEPPED DOWN BETWEEN 2 CONCRETE BLOCKS. TWISTED RIGHT KNEE.
Operator making adjustment to winch. Removed safety pin while tension was still applied to cable. Brake did not hold due to tension, winch handle spun hitting operator in the wrist & hand
Operator was attempting to release tension from a hand winch he pulled the safety pin without securing the winch handle, the brake was defective so it spun the handle, which striked the operat ors hand ors hand
EE WAS WALKING TO CONTROL HOUSE AT BC9. AS HE STEPPED ON THE PLATFORM HE STEPPED ON SOME ICE & HE FELL.
EE WAS CHECKING A SOLENOID ON A WELDING MACHINE WHEN HIS THUMB GOT CAUGHT IN THE FAN.

EMPLOYEE REACHED UNDER A DESK IN THE SALES DEPT TO PICK UP SOME PAPER AND HIS LEFT FOOT SLIPPED OUT FROM UNDER HIM CAUSING HIM TO FALL ON HIS RIGHT KNEE. HE TWISTED HIS KNEE, CAUSING IT TO SW ELL UP AND KEEP HIM FROM WALKING WELL.
