An employee's left hand became lodged between the rotating pulley and the gear box shaft on radial stacker

Employee was changing out a screen at the Wet Station and loss EE's balance and fell causing lacerations to the back of the head. Employee was placed on restrictions and received three staples making this incident an MTI.

EE was working on closing a lid on a VSI Crusher when it became stuck on descent. EE states they placed hand between lid slot and ear for the lid as they were asking the operator to raise stop and raise the lid. The lid released unexpectedly causing the EE's left middle and ring fingertips to be severed.

Employee was helping change out jaw dies in the # 1 jaw. They were in the process of taking out the cheek plate when the pad eye that was welded to the plate broke out and struck employee on his right cheek.

INJURED WAS ATTEMPTING TO FREE A ROCK THAT WAS WEDGED BETWEEN THE CABLE A SHEAVE ON THE MOTOR CRANE. HE WAS PULLING ON THE CABLE, THE ROCK FELL ON THROUGH, THE CABLE WENT DOWN AND PULLED HIS H AND IN BETWEEN CABLE AND SHEAVE. FINGER KEPT HURTING AND HE WENT TO DR 8-30-90, WAS X-RAYED AND GIVEN PRESCRIPTION FOR INFLAMMATION.
INJURED WAS STANDING OUTSIDE HIS TRUCK WITH THE DOOR OPEN WAITING TO GET LOADED WHEN A HARD GUST OF WIND HIT THE DOOR. INJURED THREW UP HIS ARM TO STOP THE DOOR FROM SLAMMING SHUT. THE JAGGED CORNER OF THE DOOR CUT HIS RIGHT FOREARM, REQUIRING SEVERAL STITCHES.

INJURED WAS USING AN AIR WRENCH TO REMOVE A NUT FROM A BOLT ON FRONT END LOADER BUCKET AND WAS HOLDING THE BOTTOM OF THE BOLT WITH THE PALM OF THE HAND. WHEN THE BOLT LOOSENED, THE HEAD OF THE BOLT CAME OUT OF THE SOCKET AND BEGAN SPINNING AT A VERY FAST RATE OF SPEED.
