EE was cutting a steel manifold from the side of 10SC001. While getting to the bottom of cutting the brackets off, the manifold broke loose unexpectedly and dropped 3 feet to the catwalk. The EE's foot got tangled in the manifold's valves and dragged EE down the catwalk. This resulted in a twisting of the EE's left ankle.

Employee was performing an inspection on the 90CR001 horizontal shaft impact crusher during a shift change. Upon inspection of the HSI Crusher, the employee found blow bars backed off and was in the process of torquing them when the employee fell approximately 4' onto the underlying 90CV001 belt. Employee received right ankle, shoulder and head injuries.
While collecting material left over from a completed shaker screen change, the employee (EE) was in the process of moving a steel plate to be taken down from the tower. During the movement, the plate shifted unexpectedly and pinched EE's finger between the plate and the catwalk. The employee reported that EE removed EE's glove and noticed bleeding from the tip of EE's finger.
Employee reported on June 28th that sometime during the evening of the 24th EE was bitten by an insect on EE's right forearm, and it progressively swelled up, EE sought medical attention. EE was taken to a clinic and treated for cellulitis. On the 29th the employee requested a hospital evaluation for continued swelling and was treated for inflammation.
The employee was heating a bolt for extraction when a piece of paint flaked off and entered employee's eye. At the time, employee was looking over the top of employee's prescription safety glasses, which were tinted due to their transition lens feature. The piece was removed with a swab and antibiotic drops were prescribed.
Employee was working on the screen at Scrubber Tower 10SC002. After putting all the tools away, the employee went down to the MCC to remove the lock and commented to a coworker EE couldn't find EE's vaping device and rushed up the tower, climbing too quickly into the screen EE's right foot got caught in the screen panel and EE fell twisting and straining it, the ankle and knee.
The employee was walking back to the loader and stepped on the loader's parking mound. EE slipped and lost balance. While falling forward EE hit left knee on the ground and put left arm out onto the loader to stop self from falling forward, hyper-extending the left shoulder and scraping left knee.
Electrician was testing a breaker, the fuse box arc flashed throwing the electrician back causing minor FA treatment burns on R wrist and neck. The employee was taken to the ER for evaluation and was released with no serious injuries or restrictions. Employee was re-evaluated on 1/24/25 for aches and pains to R wrist and R knee and was prescribed sessions of physical Therapy.

Welder was performing work on the head chute 80-CH-002 and had to install temporary steps for access inside the chute by tack welding the steps in place. After complete, steps were being removed by breaking tack welds with hammer. One of the steps rolled over and hit left corner of forehead causing small cut requiring 4 stitches.
EE was riding as a passenger in a fuel truck when the driver over-corrected the vehicle on an uneven surface, causing the vehicle to flip over to the passenger side. The passenger incurred a right shoulder rotator cuff strain.
Employee was moving extra chain out of the way from an air hoist holding a screen up. The airline fitting feeding air to the air hoist had a crimp fail and pulled loose. When the crimp failed the air filter that hangs from the air hoist, that weighs approximately 10LBS, fell approximately 18 feet striking the employee in the right arm just below the elbow.
Employee was attempting to load a return roller training frame into a wheelbarrow by self and it started slipping in EE's hand and EE lunged forward to put it in the wheelbarrow. While doing so, EE lacerated EE's right index finger in between the frame and wheelbarrow.
Employee was attempting to put an impact roller from a conveyor belt back on the frame. The roller got wedged between frame and the roller. Employee was holding the roller with one hand on each end and the roller got unstuck. The roller landed on and pinched EE's pinky finger.

After filling up a loader when employee stopped the nozzle the hose feeding the nozzle from the Mobile Fuel Truck blew out spraying EE in the face with red diesel.
The employee was pulling a 1-½" water hose under 80CV004 conveyor belt and it got stuck so they pulled harder and that's when they felt a sharp pain in their abdomen down to their groin
Employee got debris in their eye while walking around the shop area.
After power washing a piece of mobile equipment, an employee fell approximately 6' to the ground while descending a ladder. The employee suffered a bruised lower back and was given days off of work.
Employee was walking out of the flocculant building and slipped outside of the sliding barn doors. When they fell, their left forearm hit the bottom of the metal door and cut their arm 2" x 2" in the shape of a V.
While removing a shaft from crusher roll 2, a maintenance employee was tapping the shaft with a hammer when a small piece of metal flew off. The injured employee was standing ~10' away, was struck by the metal, and it became embedded on the right side of their neck. The employee was taken to the ER where the metal was removed and the employee received 3 stitches to close the wound.

Main water pump at mine had blockage. Employees of the mine along with one contractor employee were taking the pump apart to remove the blockage. Heat/steam had built up pressure in the pump unknowingly and released when the bolts were removed from the flange on the pump. Result was 2nd degree burn on 25% of body of contractor employee.
As the suction line was being removed at the Primary scrubber 10 PU 1 pump, hot steam and water was released out of the suction line injuring 3 employees and 1 contractor.

Employee was cutting and grinding metal with a torch and grinder and began to feel irritation in their eye. After employee flushed out with eye wash, irritation continued and resulted in a visit to the doctor. Nothing was removed from the employee's eye but EE was given a prescription. The employee was wearing safety glasses and a face shield while conducting the work.
Employee was climbing down off a ladder, while in the process they stepped on a rock causing them to roll their right ankle, resulting in a sprain.

Employee was cutting a pipe out of an old tank, once the final cut was made the pipe, which was under pressure gave way and struck the employee in the face resulting in a small hairline fracture laceration to the employees nose resulting in 3-4 stitches.
While employee was unloading a bucket of hydraulic fluid from the back of truck, the plastic rim of the bucket broke causing the metal handle that the employee was holding to slip. While in the process of slipping the metal handle (hook on handle) caused laceration to employee's right index finger resulting in stitches.

Employee was inside tank welding a patch over a leak. Night shift control room operator was unaware and started pumps in preparation to start plant, causing water to rush into tank while employee was inside. EE was taken to ER and was given antibiotics and kept overnight for observation. EE was not able to return to next regular scheduled shift, making this a LTI.
Employee was cutting a torch hose when employee slipped off and cut right thigh.

Employee was dismounting from the motor grader while conducting the pre-shift inspection on the equipment. As the employee came down facing forward, EE's left pinky finger caught a tire tread groove and was jammed, causing a possible dislocation and a fracture of 5th digit.
EE was working to shorten a chain on an apron feeder. When attempting to drive a connecting pin back into the chain, the pin fractured causing shrapnel to strike the EE in the forehead causing a small laceration. EE received 3 sutures to close the wound.
EE was installing the shroud on the differential and was checking shroud and gear clearance. EE's finger got caught between the gear and the shroud

On 07/25/2018 at approx. 1430 hrs, a Haul Truck Operator, fell approximately 9' to ground while attempting to dismount the Cat 777D EE was operating. EE sustained lacerations to the head and an open compound fracture of the right forearm that required surgery to repair. EE was not able to report to work on EE's next regularly scheduled shift, making this a Lost Time Injury.
EE was working to clean a section of conveyor ee was to perform maintenance work on. As ee went to pull a water hose out from under a conveyor, ee stepped backward tripping on a section of the water hose that was on the ground behind ee, causing ee to fall onto left wrist, fracturing it.
EE was performing hot work on a crusher roll, as EE stepped back to review the cut EE had made, the limestone fines found in the crevices of the roll burst outward, causing dust to enter the employee's left eye. Employee was seen by Dr. and a corneal abrasion was found and prescribed eye medication.
EE was working on a roll crusher that was being rebuilt, while attempting to free up a bolt that had been cut from a sizing segment, the pry bar ee was using slipped and struck the employee on the lower right side of the jaw, breaking a wisdom tooth. EE had the tooth extracted by an oral surgeon on 1FEB2018, sutures were used to close the incision.

EE was descending cab of Cat 992G Loader when the EE slipped on the last rung of the ladder, falling backwards onto head creating a laceration that required 4 staples to close. EE was released to work without restrictions.
While attempting to mount a Cat 777D, an employee with 2 years of mining experience had the wire rope ladder swing forward towards the vehicle causing the employee to fall downward with right foot still on the rung and landed on ee still extended left leg. As a result the employee suffered a hyperextension of the right knee.
WHILE GRINDING ON CRACK ON DISCHARGE OF DEWATERING SCREEN, GRINDER HUNG ON CRACK AND KICKED BACK; KNOCKING OFF WELDING HELMET AND HITTING EMPLOYEE'S MOUTH, CUTTING LIP.
On 3/23/2017, EE reported minor pain in left knee after stepping down 2.5' onto a conveyor from within the body of a cone crusher. On 3/29/2017 EE asked to see the Dr. prior to going on vacation, diagnosed w/ L knee MCL Sprain. On 04/05/2017, EE had follow up visit w/DR upon return from vacation and was released back to work with restrictions.
While removing the metal skirt board from the conveyor, employee's left pinky finger was caught between the skirt board in hand and chute. Employee was wearing glove for safety. Cut was immediately attended to in Emergency Room and received 4 stitches and antibiotics. No lost time days.

While lowering chain curtain into a feed chute in an aggregate silo, worker reached down to move the curtain into the slot, the curtain shifted and caught the end of his glove pulling his finger under the shaft attached to the curtain, smashing the tip of his finger off.
While removing flywheel for maintenance, a 3/4" bolt broke, hitting worker above the eye and making a minor cut requiring stitches.

Setting screen deck in place, got hung up, employee was looking to see where it was hung up at, deck suddenly slid down frame landing down on his right lower leg between deck and channel iron, was not pinned in due to the back up come-a-long being in place.
The employee was cleaning a chute of consolidated material when a large portion of material came loose and struck him in the chest. He suffered fractures to the ribs (2) and sternum.
Scrubber pump above new screen had dripped oil onto deck. Worker slipped in oil puddle landing on left shoulder.
Employee was using an impact wrench to tighten a J-bolt. His left index finger was struck by the spinning j-bolt resulting in soft tissue damage and a slight fracture. First aid treatment only. X-ray positive for fracture is the reason for reporting.
Employee was replacing an air pump in operators seat. He had the seat blocked up and the block slipped crushing his right arm.
Employee stooped down to pickup a 12 to 18" rock and felt pain in his lower back.
Employee was attempting to turn a valve when his right leg slipped causing the injury to his right knee.

Employee suffers from tendonitis in the left shoulder as a result of repetitious work related activity. Fueling 777 Cat haul trucks.
Employee sprained his ankle when he stepped on to cat walk after releasing his lanyard from a position above his head.
The employee was dragging a tow cable. He dropped the cable and it struck his right thigh causing a laceration. Sutures were required.
Employee was attempting to move around the end of a rail car, when he slipped and fell striking his head on the rail car. Height of fall was approx.3 ft. Sutures required to close wound.
A mechanic suffered a torn meniscus in the left knee when the tailgate on the service truck struck his knee while attempting to climb in the bed. The employee refused medical attention until January 20th, 2014. The employee continued his normal duties until surgery on May 13th 2014. This report is delayed due to delayed diagnosis and no medical treatment for 14 months.

Employee reported left shoulder began hurting while driving truck. Initial diagnosis was strain/sprain. MRI revealed arthritis, bone spurs, and SLAP tear. None of these conditions are a result of employees work activities. Employee was prescribed therapy beginning 11/27/12. Advised by MSHA Denver to report incident anyway.

Employee was lifting a roll of skirtboard rubber causing strain. This injury became reportable on Jan. 13th 2012, when the employee finally was able to see an orthopedic surgeon. The injury had not been medically treated or immobilized, nor had any prescription been issued and the employee continued to perform his normal duties as a leadman at the loadout.
Employee's left index finger was caught between an electric motor and motor base as the motor was being lifted by a mobile crane. The employee was trying to prevent the motor from swinging.
The employee was grinding inside a shaker when a tiny particle bounced off the side of the shaker and went behind his safety glasses. The particle was removed with a cotton swab but antibiotic drops were administered to prevent infection.
Employee was using a steel bar to clear chute of obstruction. The bar pulled through his hand cutting his glove and his left hand. The injury required sutures.
EE had been working on machine when he felt pain in right forearm. Pain has progressed and treatment was sought one week later.
EE was using a grinder and a piece of metal fell into his right eye. He was wearing required PPE and conditions were windy.
This customer truck driver attempted to exit the cab of his truck and became entangled in the seat belt causing him to fall out of his truck breaking his leg.

Employee was trying to pull a grease gun hose from a grease line hose. He was having difficulty. He exerted more force causing the two hoses to come apart. His right hand flew into a conveyor frame below the hand rail. His right index finger went into the pinch point between the roller and conveyor belt. He suffered a fractured & lacerated finger.
Employee lacerated his right thumb on a razor knife while performing repairs to a R60 Haul truck.

Drill steel fell out of carousel. Driller couldn't get it back in by himself so he had EE come help him. Pipe was stuck so the driller started the rotary head down to unstick it but it hit the pipe smashing EE's hand.
While cutting holes in a piece of belting to be used as a curtain on a dewaterings screen at the sand plant, EE was on the 7th hole and standing while cutting toward his body, the utility knife came off the belt bringing the knife across his right thigh leaving approx a 1.5 cm laceration just above his knee.
Employee was operating a dozer pushing material. As he was backing the dozer, he struck a large rock, jarring and straining his neck.
Employee was in the process of repositioning a grease line, when his left index finger (finger nail) was caught in between belt and roller. The employee received a lacaretion, requiring sutures.

Workers were performing conveyor roller replacement, when an 8' long roller slipped and slid on framework, towards worker. Worker put up his right hand and roller pinned worker between roller and column, bending right hand back onto his arm.

On 8/15 received notice from doctor that employee was diagnosed with back pain and was being treated with Chiropractic care. Employee was released to work on modified duty.
After employee finished cutting pipe he noticed small clump of grass on fire. As employee raked dirt onto fire with foot felt sharp pain in right knee. According to doctor, employee sprained knee and was issued a brace.
Employee while removing broken glass from window pane cut through gloves and lacerated his left thumb. He was taken to the local hospital where he recieved 5 stitches. He returned to work on same shift with no restrictions.
While inspecting a rail car, EE pulled right shoulder. At the time, EE did not feel anything was wrong with shoulder. EE decided to see a doctor on 4/11/05. Doctor released EE to work, no restr. & scheduled MRI. Specialist reviewed MRI, diagnosed a rotary cuff injury on 5/10/05. Employee released back to work, restricted duty, until surgery on 5/26/05.
Employee was standing on a platform observing the Primary hopper being cleaned out by the backhoe when a hinged catwalk section he had lifted and latched back to the handrails prior to, came down and landed on the employees' foot. The conditions contributing to the accident was due to the latch failing to hold the catwalk.

Employee, while assisting the ground crew get a sump pump out of a muddy area, strained his groin area. The employee was diagnosed on 11/29 as having a hernia and was scheduled to have surgery on 12/13/04. Employee continued to work with no restrictions to his job up until his surgery.
While drilling, employee wiped his (L) eye with his sleeve and got a piece of metal in it.
After exiting his personal vehicle, employee was walking across parking lot and stepped on some clothes hangers. He fell striking his left knee on the ground. This accident was not reported to his supervisor until 11/02/04.
Employee was welding on a 12" pipe in the sand plant and was grinding on root pass when the wind gusted and particles got into his eyes.
Employee was grinding and when he raised his face shield, accumulated metal shavings on the frame went into his eye.

WHILE THE MECHANIC WAS ATTEMPTING TO JOIN THE PULL-DOWN CABLE TO THE ROTARY HEAD, A WELDED RESTRAINING NUT ON THE PULL-DOWN CABLE CLEVIS FAILED AND THE ROTARY HEAD FELL APPROX. 10 INCHES BEFOR E BEING STOPPED BY THE HOIST CHAINS. THE ROTARY HEAD STRUCK THE MECHANIC'S HEAD IN THE FALL. THERE IS NO INDICATION YET WHETHER WORKER ERROR PLAYED A ROLE IN THE INJURY.

LOADER TIPP ED OVER EMPLOYEE FELL FORWARD OUT OF LOADER, PINNING HIS LEFT LEG BETWEEN LOADER BUCKET AND QUARD AT END OF RAMP.
