ON 9/26/96, EE TWISTED HIS L KNEE WHILE MOVING ON OX BOTTLE BECAUSE HE STEPPED ON A ROCK. WAS SEEN BY COMPANY DR. & RELEASED BACK TO WORK. HAD PHYSICAL THERAPY UNTIL 11-3-95 ON A WEEKLY BASIS. HAD ARTHROSCOPY SURGERY DONE ON 11-3-95, THEN RETURNED BACK TO WORK ON 11-9-95.
AFTER MOVING THE AIR TRACK DRILL IN A POSITION TO START DRILLING A SHOT, HE WALKED DOWN THE SLOPE TO CHECK THE AIR COMPRESSOR AND WHILE DOING SO, LOST HIS FOOTING AND FELL TO THE GROUND, LANDI NG ON HIS RIGHT ELBOW, THUS CAUSING THE CONTUSIONS AND ABRASIONS.

EE WAS BUILDING UP HAMMERMILL HAMMERS AND WAS PUTTING THE LAST PASS WITH HARD FACING RODS AFTER WEDDING THE HAMMER HE STOPPED LIFTED HIS WELDING MASK AND WAS STRUCK IN THE LEFT EYE WITH A HOT FLAKE OF WELDING SLAG AS THE SLAG COOLED DOWN.
