Employee was operating a haul truck and hadn't notice a loader had stop to let him go by (both haul truck and loader were going in same direction). The haul truck's catwalk, on the driver's side, came in contact with the side of the loader bucket which pushed the hand rail of the catwalk through the driver side (open) window, which hit the employee on his nose and broke it.

Employee was operating a 50ton haul truck and had started dumping a load of sand when the haul truck leaned to one side at which time the operator retracted the bed of the truck which caused the truck to jerk. The operator complained of soreness to his back and shoulder.

HE WAS HOLDING ON HAND RAILS WITH BOTH HANDS BUT HIS RIGHT FOOT SLIPPED TO THE NEXT LEVEL CAU SING HIM TO HIT HE LEG/ANKLE AGAINST THE FRAME OF THE DOZER. EMPLOYEE STATED THAT HE MAY HAVE TWISTED HIS ANKLE BUT HE WAS NOT SURE. HW WAS OK BUT HIS LEG BECAME SWOLLEN THE NEXT DAY.
