The haul truck's catwalk, on the driver's side, came in contact with the side of the loader bucket which pushed the hand rail of the catwalk through the driver side (open) window, which hit the employee on his nose and broke it.

Employee was operating a 50ton haul truck and had started dumping a load of sand when the haul truck leaned to one side at which time the operator retracted the bed of the truck which caused the truck to jerk. The operator complained of soreness to his back and shoulder.
Crusher liner fell on employee's left leg causing pain in left knee. This accident happened on 5/23/06 but employee did not go out on injury until 5/27/06.

EMPLOYEE WAS STEPPING OFF HIS TRUCK ONTO A PLATFORM WHICH HAD CONTROLS USED TO FILL TRUCK FROM A STORAGE BIN INTO THE TRUCK TRAILER. THERE WAS A GAP AND EMPLOYEE FELL TO THE GROUND HURTING HIS SHOULDER.

WHILE DRIVING BOTTOM DUMP PIT TRUCK EE KEPT BUMPING HIS LEFT ELBOW ON THE SIDE OF HIS DRIVERS DOOR, UNTIL ELBOW BECAME SORE AND THE EE REQUESTED MEDICAL ATTENTION.
WHILE EXITING THE CAB OF A DOZER, HE WAS STEPPING DOWN WHENA GUST OF WIND KNOCKED HIM OFF BALANCE. HE WAS HOLDING ON HAND RAILS WITH BOTH HANDS BUT HIS RIGHT FOOT SLIPPED TO THE NEXT LEVEL CAU SING HIM TO HIT HE LEG/ANKLE AGAINST THE FRAME OF THE DOZER. EMPLOYEE STATED THAT HE MAY HAVE TWISTED HIS ANKLE BUT HE WAS NOT SURE. HW WAS OK BUT HIS LEG BECAME SWOLLEN THE NEXT DAY.
