Miner was hammering in a choker plate for material feed, missed the plate and struck EE's R-index finger. Miner was wearing leather gloves at the time and sustained a fracture to EE's fingertip.

Cut left index finger during maintenance. Hand was placed on gravity take up guide pole, 966 loader bucket that was supporting the counter-weight bled down hydraulic pressure causing the counter-weight to lower and finger got pinched by the guide pole sleeves.

Jaw crusher plugged filling the surrounding area with material, EE & 2 workers removed the material they used a crane & sling operation because some of the damaged deck could be a trip hazard they removed 2 sections ee was standing with his back to 1 hole he then turned & lost his balance fell approx. 12' onto his side, ee had a bad cut over his right ear

EE was cleaning up the material that had spilled around the sand screws discharge collecting box with a shovel and in performing this task he slipped and lost his footing and fell thus cutting his right forearm with a 2.5 cm cut. He wasn't sure what actually cut his arm, he was released to come back to work from the clinic but he decided he needed a day of rest.
