Employee stated through the course of duties changing screens on a shaker deck on 5/22/2020 that employee's knee tightened up. Employee thought it would be fine, but 5-hours into the shift on 5/23/2020 employee reported the injury.

Employee was attempting to un-hook a com-a-long from a chute and cut his arm on the chute.
Employee noticed a piece of metal that was on a conveyor belt (that just had gone through a crusher) and went to pick it up, the metal was hot and burnt two of his fingers.

EMPLOYEE WAS DOING CLEAN-UP IN THE AREA OF #2B CONVEYOR (STAND AND CRUSHER UNDER BELT), WHEN A 4" - 8" ROCK CAME FROM ABOVE AND STRUCK HIM ON THE RIGHT FOREARM CAUSING CONSIDERABLE SWELLING AN D PAIN. HE WAS TAKEN TO DOCTOR.
