EMPLOYEE WAS LIFTING STEEL WITH THE BOOM TRUCK. WHEN HE STARTED TO LIFT THE STEEL, IT SLID BACK, PINCHING FINGER INTO TRUCK.
SPEED OF PLANT WAS INCREASED APPROX 25% ABOVE NORMAL WHICH CREATED EXTRA STRAIN ON THE LOADER OEPRATOR'S WRIST WHILE MANAGING FEEDING THE PLANT.
