10-05-07-Employee complained of pain in the right shoulder and was given first aid. On 11-29-07 employee notified employer that the shoulder wasn?t getting better. Employer sent employee to a physician for further treatment at the earliest available appointment on 12-05-07. The diagnoses of bursitis and tendonitis required physical therapy and as such became a recordable injury.
When EE climbed the loader ladder to enter his cab, he missed the handrail, slipped and fell to the ground onto his right hand. His right wrist broke as his hand struck the ground.

Walking down the ramp, operator stepped on a cobble and twisted his knee.
Employee was working on the dust control sandvik crusher deck. He attempted to pull on belt to line up bolt. While tightening up the fitting with a wrench, he strained his lower back. He returned to work on 3/16/06 and has worked every day since until 6/16/06. He reported on 6/15/06 to the office, that he has been going to physical therapy since 6/7/06.
