Employee setting up backhoe to begin clearing area near tunnel when EE felt discomfort in left eye. Employee continued to work for some time before attempting to remove object with eye wash. When object did not come out, employee was taken to clinic where metal shard was removed.

After taking break miner was walking to gather tools to complete a housekeeping project. Miner slid left hand into pocket (while wearing gloves), where open pocket knife cut the top of miner's hand just above the thumb.

A scraper operator was headed eastbound through the bottom of the pit when the back end of the scraper lost traction. The scraper spun around and eventually grabbed traction as it slowed down. Momentum caused the scraper to tip onto it's side.

While replacing bin pulley assembly, employee was removing a bolt with a 1/2 impact and a 15\16 ratcheted box end wrench. The wrench spun, trapping finger between the wrench and the bracing resulting in the loss of the tip of left ring finger.

While removing the retarder on a 633 mega scraper the retarder valve fell and struck the employee in the eye area. The employee was wearing safety glasses at the time of the incident. The angle of the falling valve knocked the glasses off and the employee got cut by the sharp metal of the valve. Employee sustained two lacerations on outside of right eyelid.

Employee was exiting skid steer and employee was standing on the right tire. When employee stepped down with right foot, employee's left foot got hung up in the tire tread causing pain in the left knee.

The employee's job is to grease and lube moving parts at the rock plant on a daily basis. This is a repetitive motion type of injury with a cumulative affect.

Employee's loader wouldn't start so he climbed down to check the battery box and move the cables. He then decided to climb back up on the loader using only the hand rail next to the engine compartment. He lost his grip and fell on his back. The operator didn't report the incident to management until June 17th, 2011.

Employee was removing timing belt / pulley from CA Screen #2 when his left hand got caught between the pulley and the belt.

Employee complaining of on-going pain, possibly due to carpal tunnel syndrome.

After driving a pry bar into the packing gland of a water pump to loosen the packing, the employee tapped the pry bar to remove it and the bar sprang out and hit him in the forehead.

Employee was cleaning inside of driveover feeder at Thoming pit using a demolition hammer. While he was cleaning the side walls he strained his right elbow and arm. His fingers began to go numb. The employee reported it to his supervisor but said that he did not require medical treatment.

Employee was removing liners with a pry bar in the rodmill. The liner was in the 10 O'clock position. When it broke loose it fell to the bottom of the barrel at approximately the 4 O'clock position and hit employee on the foot above the protective toe of his boot.

Employee claims he was injured while operating scraper on bumpy road.

While moving a piece of steel by hand in the steel yard, employee twisted his back.

Employee was cleaning out a screen and hit his finger with a hammer

A verbal incident turned physical. One person grabbed the other person's hand and twisted it.

Changing bit in electric drill resulted in laceration to web of right hand.

EE WAS GRINDING WITH A 4-1/2" HAND GRINDER AND THE GRINDER KICKED BACK AND STRUCK EE IN THE FACE.

WHILE INSTALLING A GUARD, EE HIT HIS LEFT ELBOW ON A UNISTRUT THAT WAS HANGING DOWN.

EE WAS SITTING ON THE SCREEN DECK HE TURNED TO PUT HIS TOOLS IN A TOOL BOX AND PULLED SOMETHING IN HIS BACK.

EMPLOYEE WAS CUTTING SOME NOTS OFF WITH CUTTING TORCH IN A DISCHARGE CHUTE. A FIRE STARTED AND IN THE EMPLOYEE'S HASTE TO EVACUATE, HE BUMPED INTO THE FEED CHUTE AND BRUISED HIS RIBCAGE.

WHILE SITTING ON A 16" PIPE EMPLOYEE WAS TRIMMING BELTING TO FORM A SEAL AROUND PIPE. AS HE WAS MAKING HIS LAST CUT THE UTILITY KNIFE CAME THROUGH THE BELT EASIER AND FASTER THAN HE WAS EXPECT ING AND WENT ACROSS HIS RIGHT LEG CUTTING IT OPEN ABOUT 3".

BARRING DOWN ON BUILD-UP ON THE INSIDE OF A CONE CRUSHER MAIN FRAME. LARGE PIECE DISLODGED LEVERAGING BAR INTO THE RIB CAGE.

AS EE WAS CUTTING PLATE WITH A TORCH SCRAP PIECEFELL & CUT RT THIGH

ATTEMPTING TO LOOSEN BOLT A V TRAP STRAINER, BOLT RELEASED, SOCKET SLIPPED OFF BOLT HEAD. RIGHT ELBOW STRUCK SCREEN CHUTE. INFECTION SET IN WITH NO EVIDENCE OF SKIN RUPTURE.

EMPLOYEE WAS OPERATING BOBCAT AND FELT PAIN IN NECK. IT GOT WORSE WHEN HE LIFTED A HAMMER PRE-PARING TO HIT A ROLLER.

EMPLOYEE WAS HOLDING BOLT WITH WRENCH WHILE USING IMPACT ON NUT. WRENCH SPUN LOOSE & STRUCK HIM.

EMPLOYEE WAS REMOVING EXCESS MATERIAL WITH SHOVEL FROM AROUND ROCK PLANT.

SETTING UP TIMBER CRIBBING TO SUPPORT TRAILER ON-NEXT DAY NECK SORE & ALSO BACK.

WHEN EXITING PARTS VAN EMPL MISSED STOP & FELL.

FIXING DISCHARGE CHUTE OFF AC SCREEN WITH NEW LINER 6' X 4' AND SLIPPED AND FELL ON LEFT BUTTOCKS.

NEW BELT WIPER STILL HOT AND HE PICKED IT UP LEFT HAND BLISTER INDEX AND MIDDLE FINGERS ONLY.

LABOR WORK INSIDE FEEDER AND HAND PICKING ROCKS UP OFF FLOOR AND A ROCK FELL ON RIGHT INDEX FINGER AND BRUISED FINGER WHICH WAS SWOLLEN.

REPAIRING SCREEN PANELS & TACKING NUTS - LIFTED WELDER DOOR AND PUT RT HAND ON THROTTLE UNCAGE & PUSHED HAND INTO FAN BLADE.
