Employee was preparing a conveyor for transport, when EE's thumb was caught in between the folding legs.

Miner was unloading material alone and was found unresponsive. After being on life support for one week, miner passed away from a brain aneurysm. We didn't feel that the aneurysm was a reportable event/injury but we are submitting the form now out of an abundance of caution.
