Caught in, under or between a moving and a stationary object

The truck driver was dumping a load of pit run and the tailgate jammed and would not latch. The driver went to clear the debris from the latch and it closed on EE's finger requiring 5 stitches.

5:30 pm fueling loader without operator knowing that he was fueling loader. Some truck came into pit. Operator got in the loader and started to take off. The hose was in. He tried to remove hose and fractured his thumb.

Fall from ladders

Slipped off ladder from about 2'-3' high, falling on back causing a back strain or torn muscle.

Stepped in low spot in the ground causing a twist to knee.
