Employee was running WA600 Loader at face when they felt intermittent pressure in left shoulder. Reported injury 10/3/2022, no lost time for injury, employee worked full time at full duty and was released from medical care 11/18/2022. Employee was expected to return from layoff when employee decided to get an MRI for shoulder. This led to surgery for which employee is recovering.
Miner was attempting to flip crusher head nut on table when the nut slipped off the table grabbing the arm and straining forearm.

Miner determined repair was needed in a chute, the miner descended catwalk of conveyor belt towards 5' vertical ladder which lead to the ground. Instead of turning EE's body and maintaining three points of contact, the miner stepped in between the top rung of the ladder and the end of the catwalk, toppled over backward and had leg/foot caught between the ladder and the catwalk.

Employee was attempting to clear stone chute that was backed up. Employee grabbed a bar and while in the back swing to strike down on the stone, the employee smashed a hand between the end of the metal bar and the metal structure of the screen tower. The result of the impact was a lacerated finger and minor fingertip fracture.

Employee was bending down to start wiring a panel, upon attempting to go from a bent over position to a fully upright position while twisting, the employee felt a sharp pain and heard a pop from employee's lower back. The employee strained employee's muscle, fascia and tendon of employee's lower back.

Haul truck driver exited CAT 773 Haul Truck to relief himself while waiting to unload haul truck into primary crusher. Employee returned to the haul truck and while climbing back up the haul truck, he lost his footing and fell off ladder approximately half way up the ladder. Employee sprained right wrist in fall to the ground.

Driller, was doing maintenance on his drill, slipped off a step and hit the handrail with his left side (ribs). I had an employee drive him to occupational health facility. X-rays were negative, just bruising. Tried to work through it, first lost day to workers comp 8/28/10, today. He has a Dr.'s appt 9/2/10. Restricted duty, lost work days.
Driver has complained about air conditioning in his truck not being adequate. Necessary repairs have been made to unit.

WHILE CLEANING OUT A CONCRETE MIXER DRUM USING A JACKHAMMER, ONE PIECE DISLEDGE ABOVE HEAD & FELL STRIKING HEAD KNOCKING OFF HARD HAT & CUTTING HIS HEAD. THE CUT REQUIRED 5 STITCHES WHICH ARE CLOSE TO THE HARD HAT LINE MAKING IT IMPOSSIBLE TO WEAR HARD HAT SO LOST TIME HAS RESULTED.
INSTALLING NEW WIRE CLOTHS ON A SCREEN--IT WAS NECESSARY TO INSERT A PINCH BAR INTO THE BOLT HOLE TO PUSH THE WIRE CLOTH DOWN. WHILE WORKING OUTSIDE, THE BAR CAUGHT THE DRAW PLATER ALSO, PINCH ING THE EE'S FINGER.
EMPLOYEE WAS CHANGING ROLLERS ON THE CONVEYOR BELT FROM THE LIFT. WHEN HE WAS REMOVING ONE ROLLER IT CAME LOOSE OFF IT'S BRACKET. CAUGHT THE ROLLER ON AN AWKWARD ANGLE WHICH CAUSED A RADIAL FR ACTURE OF HIS RIGHT ELBOW.

AS HE WAS PREPARING TO DUMP THE BUCKET, THE VERTICAL SLOPE G AVE WAY. THE RIGHT FRONT TIRE WAS BURIED & THE RIGHT WINDOW WAS BROKEN. THE EE RECIEVED A CUT ONRIGHT MIDDLE FINGE FROM THE BROKEN GLASS.
EMPLOYEE TRIED TO TAP A SIDE SKIRT BACK INTO PLACE ON A CONVEYOR. THE BAR HE USED HIT THE CONVEYOR SNAPPING IT UPWARD PINCHING HIS FINGER AND THUMB AGAINST A BRACE. THE SKIN AND NAIL WERE AF FECTED.
EE WAS STANDING ON TRACK OF AIR DRILL AND WHEN HE STEPPED DOWN HE STEPPED ON A LOOSE STONE. THE STONE MOVED CAUSING HIM TO FALL ON HIS LEFT FOREARM AND SHOULDER.
EE WAS REMOVING BOLTS FROM STONE CRUSHER WITH RATCHET FELT PAIN IN RIGHT SHOULDER.
EE WAS WORKING ON GEAR BOX OIL PAN ON SCRAPER. WHILE INSTALLING DRAIN PLUG, HE REACHED UP TO SCREE THE DRAIN PLUG IN, WHEN MATERIAL FELL FROM AREA OVERHEAD INTO HIS LEFT EYE.
EMPLOYEE WAS INSTALLING TIRES ON TRUCK AND IN THE PROCESS OF INSTALLING A RIM SPACER ONTO HUB. HE WAS USING A SLEDGE HAMMER TO TAP SPACER ONTO HUB. WHILE SWINGING THE HAMMER WITH HIS LEFT HAND ON THE HANDLE END AND HIS RIGHT HAND ON THE HANDLE ABOUT 6" FROM THE HEAD. HE MISSED THE SPACER & PINCHED HIS RIGHT MIDDLE FINGER BETWEEN HAMMER HANDLE & SPACER CUTTING THE SKIN & TEARING NAI

EMPLOYEE WAS ADJUSTING THE CLUTCH OF A TRUCK WHEN HE CAME UP FROM UNDER THE TRUCK, HE FORGOT THAT THE CAB DOOR WAS OPEN AND CUT HIS HEAD. REQUIRING 5 STITCHES.
EMPLOYEE WAS TRYING TO LOOSEN UP A BEARING, WHENHE ACCIDENTLY HIT PART OF THE BEARING WHICH CAUSED A PIECE OF METAL TO PENETRATE HIS LEFT HAND BELOW HIS THUMB.
ROBERT WAS POKING STONE IN A CRUSHER. THE CHUTE WAS PLUGGED UP, SO HE STUCK THE POKER IN THE CHUTE TO BREAK A HOLE IN THE STONE, WHEN HE DID THIS, THE POKER GOT STUCK, AND WHEN HE TRIED TO PU LL IT OUT, HE PULLED A BACK MUSCLE.
