Employee was in a boat, pulling on electrical cable that was stuck in the mud. Electrical cable is utilized to power dredge. While tugging on the cable employee strained their back.

While loading 3/4" cable into a work boat, the miner lifted several bound wraps of cable over the handrail and let go. When the loose end of the cable went over the rail, it whipped up and struck the miner in the right elbow. This resulted in a contusion on the elbow and tingling in the right hand. The miner returned to work, but with medical restrictions on right arm.

While washing down the plant, the operator had water spray come under the side of safety glasses. Operator wiped sandy water from eye, but a grain of sand got into left eye, causing an abrasion. Employee was taken to clinic where sand grain was removed. Antibiotic drops were prescribed and operator was released back to work with no restrictions.

While walking at sand plant, employee slipped on algae that had grown on concrete floor and fell. The fall resulted in a sore wrist, later diagnosed as a sprained right wrist and sprained hand.

Employee was removing wooden spool from stand. Upon removing the center pipe, spool reel fell and cut the tip of his finger.

Employee was rolling up an air hose and descending from a staircase. While on the last step, he stepped back forgetting he was on the stair and fell onto his backside and then on his lower back.

Employee began complaining on July 5th of respiratory problems that he felt were caused by inhaling smoke fumes and dust during the cleaning of a structure cleared) that caught fire on July 2nd.

Employee was stepping onto the conveyor walkway when his foot slipped off. He then grabbed the handrail which spun him around and twisted his back.

Stepped on pipe. Foot slipped, twisted left ankle.

Employee was stepping up a 3' step ladder. As he stepped onto the 2nd step he felt a pop in his left knee.

OPERATOR, CHANGING A FUSE, RECEIVED AN ELECTRIC SHOCK. HE DID NOT USE A HOT STICK OR GLOVES. HE DID NOT RECEIVE ANY INJURIES BUT WAS SENT TO THE DOCTOR TO BE CHECKED OUT. HE DID NOT USE LOCK-OUT TAG-OUT PROCEDURES.

EMPLOYEE GETTING INTO BOAT TO GO TO DREDGE AT 5:00 AM AND STEPPED DOWN. STEPPED ON CHAIN AND TWISTED RIGHT ANKLE.

EE WAS CHECKING BOOSTER GAUGES (AS REQUIRED EVERY 2 HOURS). RAIN HAD WASHED SAND FROM UNDER GRASS CAUSING A HOLE. EE DID NOT SEE HOLE. STEPPED IN HOLE TWISTING ANKLE.

2 EMPLOYEES WERE CARRYING SHAKER SCREEN UP TO SHAKER. EMPLOYEE INJURED SAID HE HURT HIS BACK.

EMPLOYEE WAS HELPING TO REPLACE A SECTION OF PIPE ON THE DISCHARGE LINE OF DREDGE. AS HE WAS REMOVING A FLANGE HE CAUGHT HIS FINGER BETWEEN THE PIPE AND FLANGE.

INJURED EMPLOYEE WAS REPORTING TO WORK IN THE MORNING. HE WAS PUSHING THE WORK BOAT, USED TO ACCESS THE DREDGE, AWAY FROM THE SHORE AND STRAINED HIS BACK.

EMPLOYEE WAS RE7RIEVING THE DRILL AUGER AND WAS USING THE FORK TO HOLD THE AUGER TO KEEP IT FROM FACE DOWN THE DRILL HE NEGLECTED TO REMOVE THE FORK BEFORE ENGAGING THE ROTATING ELEMENT WHEN THE AUGER TURN9D THE HANDLE OF THE FORK STRUCK HIM ON THE RT SHIN CAUSING A BRUISE AND LACERATION
