THE EE STATED THAT WHILE LOADING TIMBERS ONTO A SCOOP HE PULLED HIS RIGHT ARM. LATER IN THE SHIFT HE NOTICED THE PAIN IN HIS ARM WAS GETTING WORSE. HE WENT TO THE HOSPITAL AND WAS TOLD THAT HE BROKE A METAL PLATE THAT HAD BEEN PUT IN HIS ARM PRIOR TO BRING EE AT THIS MINE.
EE TRIPED OVER A PIECE OF BELT STRUCTURE WHILE MAKING A BELT MOVE.
EMPLOYEE STATED THAT ON 3-31-94 WHILE WORKING ON HIS KNEES. THIS CAUSED HIS KNEES TO SWELL, REPORTED TO MANAGEMENT ON 4-4-94.
