ALLEDGES HE REINJURED THE KNEE ON 8/24/ 98 WHILE SQUATTING. WE WERE NOTIFIED BY LETTER DATED 2/1/99 THAT WORKERS COMP CLAIM WAS RE-OPENED ON 1/25/99. HE HAS BEEN OFF WORK SINCE 1/27/99.

A PIECE OF TREE LIMB CAME PUT OF LOADER BUCKET AND HIT LOADER WINDOW BREAKING WINDOW AND STRIKING OPERATOR IN LEG CAUSING SMALL CU T TO LEG OPERATOR WENT TO CLINIC TO GET CHECKED.NO STITCHES WERE REQUIRED HE RETURNED TO WORK ON NEXT SCHEDULED SHIFT.
