EE WAS USING THE BOOM ON HIS MECHANIC'S TRUCK THE LIFT CABLE ON THE BOOM BROKE CAUSING THE BOOM TO FALL STRIKING HIM ACROSS THE BACK. THIS KNOCEED HIM TO THE GROUND RESULTING IN 2 BROKEN RIBS 1 FRACTURED VERTABRAE, CUTS & ABRASIONS TO THE RIGHT OF HIS FAC
EE WAS LOADING SHOT ROCK WITH ASSISTANCE OF DOZER. THE DOZER CAUSED A LARGE ROCK TO SLIDE INTO THE LOADER BUCKET STOPPING THE LOADER SUDDENLY. THIS RESULTED IN A WHIPLASH EFFECT TO THE OPERATO R.
EMPLOYEE WAS SCALING HIGHWALL WITH DOZER. AS HE BACKED UP THE DOZER DROPPED OVER A LARGE ROCK CAUSING THE OPERATOR TO STRIKE THE ARMREST WITH HIS LOWER BACK.
EMPLOYEE WAS CLEANING TRACKS ON DOZER WITH SPADEHE STRAINED HIS SHOULDER WHILE DOING THIS.

ORIGINALLY CLAIMS TO HAVE STRAINED HIS LEFT KNEE ON 2/6/98, DIDN'T SEEK MEDICAL TREATMENT UNTIL 2/17/98. ALLEDGES HE WAS STANDING AWKWARDLY FOR 2 HOURS. ALLEDGES HE REINJURED THE KNEE ON 8/24/ 98 WHILE SQUATTING. WE WERE NOTIFIED BY LETTER DATED 2/1/99 THAT WORKERS COMP CLAIM WAS RE-OPENED ON 1/25/99. HE HAS BEEN OFF WORK SINCE 1/27/99.
