Operator was running an open cab stand on skid steer and was not paying attention to surroundings and hit their hard hat on a guard of a conveyor. The operator went to the emergency room and was cleared by doctor to return on 3/2/26 missing one day of work.

Clear dry day, employee went to exit the loader and missed the first step coming down. EE fell back landing on butt, back, & hitting head. EE didn't lose conciousness, got up and walked to where others were at. Went to an urgent care for eval and was sent to ER for CT. Only severe bruising was noted. Light Duty for three days.

Work was being done on a conveyor approx 8ft above ground. The work done was overhead for several hours. While replacing a roller the employee felt a pop in EE's right shoulder while prying roller in place. EE immediately reported it at the time. EE let me know 2 wks later that there was still discomfort and received medical evaluation. results: limited use till see specialist.

While removing an impact bar from the crusher and placing it on a pallet, the bar shifted and rolled onto EE's left metatarsal. This bruised at point of impact. Site tenderness was noted but motion was good. We treated as a bruise injury and EE finished the workweek. It was observed Monday to still be tender and EE went to see a doctor Tuesday. Xray revealed a bone fracture.

Employee was preparing to wash spillage from under a conveyor. A new nozzle that was being used was 1.5" thread and so was the mating threads. The threads were close enough to engage and tighten. However, the unknowing different thread type did not hold pressure resulting in the hose blowing free and striking the employee in the upper lip.

Employee was lifting nitrogen charging service tool box from the side box of service truck. The tool box weighs about 30 pounds and the side box shelf was about chest high. Employee felt a strain in right shoulder. Follow up with same day medical evaluation indicating strain of muscle and tendons of the rotator cuff.

EE was operating the mobile fuel truck into position for fueling a Cat dozer & the excavator. As he was backing the fuel truck, the brakes failed & he backed into the right front of the dozer blade. The fuel truck sustained a small dent in the right rear bumper. EE stated on 11/7 his neck & upper back hurt & he was seeking chiropractor treatment. The ee is still being evaluated.

Employee was using a long pry bar to clear bridged material in a conveyor transfer chute. The bar pinched his left index finger between the bar and the transfer chute housing resulting in a laceration. Sutures were required creating limited use of the left hand.

Employee was using a long bar to unlodge rocks in a transfer chute from the primary plant's scalping screen. Employee reported feeling a strain or "pop" in neck or upper back & the feeling of arms & legs going numb. Contacted EMS & transported employee to hospital for evaluation. MRI revealed no deformity & released him pending further evaluation by specialist.

EE was removing empty conveyor belt spool from the installation hanger. His left little finger was pinched between the hanger frame and the spool shaft. Medical treatment, including sutures, were required to close the laceration. The EE was wearing leather gloves at the time of the accident.

ON OCT. 22 THE RESULTS OF EMPLOYEE'S ANNUAL HEARING TEST WERE RECEIVED FROM AN INDEPENDENT FIRM CONTRACTED TO DOCUMENT THE HEARING CONSERVATION PROGRAM (HCP). THE ANNUAL TEST CONDUCTED ON OCT. 6 SHOWED A REPORTABLE HEARING LOSS IN HIS RIGHT EAR COMPARED TO THE OCT. 2000 BASELINE. THIS EMPLOYEE OPERATES A CAT 777D HAUL TRUCK 40 HRS A WEEK.

LATER THAT EVENING & NEXT MORNING EXPERIENCED PAIN. 0N 10/12/99 DIAGNOSED COMPRESSION FRACTURE OF SPINE.

A PIECE OF METAL WAS DISLODGED BY THE IMPACT AND STRUCK THE EE IN THE UPPER LEFT ARM, PENE TRATING TWO LAYERS OF CLOTHING AND LODGING 1/4" TO 3/4" BENEATH THE SKIN. EE WAS TAKEN TO UNITY HEALTH CLINIC AND THEN TO A PLASTIC SURGEON, WHO REMOVED THE OBJECT FROM THE ARM AT HIS OFFICE.
