Dupo Plant #9: 262 MSHA citations Metal/Non-Metal

Employee was moving a mechanical lever to change products and felt a strain in the gut area. No action was taken at this time as felt it was a strain from exertion. Currently employee is working at full capacity with some discomfort.

Employee was assisting in changing screens stepped wrong and knee popped. Was taken to urgent care, treated as sprain, Ice with no restrictions was prognosis.

Clear dry day, employee went to exit the loader and missed the first step coming down. EE fell back landing on butt, back, & hitting head. EE didn't lose conciousness, got up and walked to where others were at. Went to an urgent care for eval and was sent to ER for CT. Only severe bruising was noted. Light Duty for three days.

While climbing down from a screen the employee slipped on some loose material and started to fall catching self. In catching self felt a pop in neck and shoulder area. No medical treatment was sought at this time.

Work was being done on a conveyor approx 8ft above ground. The work done was overhead for several hours. While replacing a roller the employee felt a pop in EE's right shoulder while prying roller in place. EE immediately reported it at the time. EE let me know 2 wks later that there was still discomfort and received medical evaluation. results: limited use till see specialist.

While removing an impact bar from the crusher and placing it on a pallet, the bar shifted and rolled onto EE's left metatarsal. This bruised at point of impact. Site tenderness was noted but motion was good. We treated as a bruise injury and EE finished the workweek. It was observed Monday to still be tender and EE went to see a doctor Tuesday. Xray revealed a bone fracture.

Employee was preparing to wash spillage from under a conveyor. A new nozzle that was being used was 1.5" thread and so was the mating threads. The threads were close enough to engage and tighten. However, the unknowing different thread type did not hold pressure resulting in the hose blowing free and striking the employee in the upper lip.

While lifting a battery out of a pickup to install in a haul truck, worker strained right shoulder.

While swinging a sledge hammer to un choke a rock chute the worker hurt left shoulder on 10/18/2017. On 10/30/17 went to doctor. On 12/4/17 started therapy. On 1/30/2018 first day lost to have surgery.

While servicing a vibrating screen at Dupo Quarry, the worker struck right knee against the corner of employee's parts crate fracturing patella.

Employee was lifting nitrogen charging service tool box from the side box of service truck. The tool box weighs about 30 pounds and the side box shelf was about chest high. Employee felt a strain in right shoulder. Follow up with same day medical evaluation indicating strain of muscle and tendons of the rotator cuff.

EE was operating the mobile fuel truck into position for fueling a Cat dozer & the excavator. As he was backing the fuel truck, the brakes failed & he backed into the right front of the dozer blade. The fuel truck sustained a small dent in the right rear bumper. EE stated on 11/7 his neck & upper back hurt & he was seeking chiropractor treatment. The ee is still being evaluated.

Employee was using a long pry bar to clear bridged material in a conveyor transfer chute. The bar pinched his left index finger between the bar and the transfer chute housing resulting in a laceration. Sutures were required creating limited use of the left hand.

Employee was using a long bar to unlodge rocks in a transfer chute from the primary plant's scalping screen. Employee reported feeling a strain or "pop" in neck or upper back & the feeling of arms & legs going numb. Contacted EMS & transported employee to hospital for evaluation. MRI revealed no deformity & released him pending further evaluation by specialist.

Employee was relocating 15 # and 50 # packages of explosives in a drop-trailer type 5 magazine. Employee felt a strain in left shoulder and was first evaluated by a physician on 4/4/2007. Physicain confirmed strain and prescribed physical therapy.

EE was removing empty conveyor belt spool from the installation hanger. His left little finger was pinched between the hanger frame and the spool shaft. Medical treatment, including sutures, were required to close the laceration. The EE was wearing leather gloves at the time of the accident.

Employee was hand shoveling in the primary crushing area for spillage clean-up. Uneven ground conditions allowed him to slip and twist left knee. The doctor's first evaluation and MRI on 01/25/07. The orthopedic doctor follow up on 02/06/07 prescribed surgery to repair.

EE LOST FOOTING ON CONVEYOR CATWALK. HIS LEFT HAND WAS HOLDING HANDRAIL AND RIGHT HAND HOLDING GREASE GUN. WHILE TRYING TO RESTRICT FALL, THE FORCE OF EE'S WEIGHT OVERPOWERED HIS LEFT ARM STRENGTH. EE'S LEFT BICEP WAS RUPTURED AND REQUIRED SURGERY TO REPAIR.

ON OCT. 22 THE RESULTS OF EMPLOYEE'S ANNUAL HEARING TEST WERE RECEIVED FROM AN INDEPENDENT FIRM CONTRACTED TO DOCUMENT THE HEARING CONSERVATION PROGRAM (HCP). THE ANNUAL TEST CONDUCTED ON OCT. 6 SHOWED A REPORTABLE HEARING LOSS IN HIS RIGHT EAR COMPARED TO THE OCT. 2000 BASELINE. THIS EMPLOYEE OPERATES A CAT 777D HAUL TRUCK 40 HRS A WEEK.

Operator was in progress of pre-shift inspection of a motor grader, while climbing onto engine compartment access platform, operator strained his mid-back region. Orthopedic surgeon prescribed therapy on 6-9-04 medical evaluation.

EE WAS REMOVING BOLT-ON CUTTING EDGE FROM BUCKET OF FEL. HE WAS USING BARE HANDS AND 1" IMPACT TO REMOVE THE PLOW BOLTS. HIS LEFT HAND WAS ON HEAD OF PLOW BOLT WHEN IT TURNED UNDER THE FORCE O F THE IMPACT. HIS LEFT INDEX FINGER SUSTAINED A LACERATION REQUIRING SUTURES.

ON 01-08-03 EE REPORTED BACK PAIN FROM HIS BODY SHIFTING IN OPERATORS SEAT WHILE DRIVING HAUL TRUCK. AN EE SCHEDULED MEDICAL EVALUATION WAS FIRST PERFORMED ON 1-24-03. MEDICATION WAS PRESC RIBED AND A FOLLOW -UP MRI PERFORMED ON 1-30-03 FURTHER MEDICAL TREATMENT IS PENDING RESULTS OF MRI.

EMPLOYEE SLIPPED ON PATCH OF ICE ON GROUND LEVEL WALKWAY, BUT DID NOT FALL. EMPLOYEE FELT STRAIN IN LOWER BACK, BUT DID NOT SEEK MEDICAL EVALUATION UNTIL 02/28/03. THE X-RAYS & EVALUATION INDI CATED PINCHED NERVE. EMPLOYEE BEGAN THERAPEUTIC TREATMENTS ON 03/03/03.

SCALE ATTENDANT GOT UP FROM COMPUTER WORK STATION AND TRIPPED ON CHAIR MAT. EE FELL TO FLOOR STRIKING LEFT KNEE ON CONCRETE FLOOR. LEFT KNEE CAP WAS FRACTURED.

EE WAS ASSEMBLING NEW STEEL INTO A GIARD UTLIZING THE WELDING TABLE AS A WORKBENCH. THE EE WAS WORKING ON THE CONSTRUCTION UNDER THE WELDING TABLE OVERHANG. WHEN EE RAISED UP TO A STANDING POSITION, UNDER HIS OWN POWER, HIS LEFT SHOULDERSTRUCK THE WELDKING BENCH.

THE PLANT WAS SHUT DOWN TO PERFORM A MAINTENANCE REPAIR ON SCREEN #11, 6' X 12' PIONEER 3-DECK SIZING SCREEN. THE REPAIR INVOLVED DISASSEMBLY OF THE POWER BEARING HOUSING. AFTER THE EQUIPMENT WAS LOCKED OUT, HE ACCESSED THE INTERNAL SCREEN FRAME THROUGH AN ACCESS OPENING IN THE PORDUCT CHUTES. DURING THIS ACCESSING, HE STRAINED HIS RIGHT ARM/SHOULDER WHILE PULLING HIMSELF INTO POSI

WHILE WALKING TO THE BREAK ROOM, EE TRIPPED ON A PLASTIC TRASH BAG AND FELL TO THE GROUND. HE SUSTAINED LACERATIONS AND ABRASIONS TO HIS RIGHT WRIST AND ELBOW. A WALKWAY GRATING USED FOR BOOT SCRAPING WAS PRESENT IN THE FALL AREA AND CONTRIBUTED TO HIS INJURIES. FIRST AID WAS ADMINISTERED AND EE WAS TRANSPORTED TO CLINIC WHERE HE REC'D SUTURES IN HIS WRIST & ELBOW, XRAYS, AND SHOT.

EE WAS OPERATING A WATER TRUCK. HE WAS THEN TOLD TO HAUL OVERBURDEN MATERIAL. AT APPROX 11:45 A.M. HE WAS TOLD TO OPERATE THE WATER TRUCK THROUGH THE DINNER PERIOD & REMAINDER OF THE DAY. HE D ROVE HIS PERSONAL VEHICLE, A JEEP WRANGLER, TO THE WATER TRUCK. AS HE WAS GETTING OUT OF JEEP HE FELL STRIKING HIS HEAD ON THE GROUND. HE WAS UNCONSCIOUS BREIF PERIOD AND TAKEN TO HOSPITAL.

EE OPERATING ENGINEER, WAS DIAGNOSED AS HAVING WORK-RELATED LEFT CARPAL TUNNEL SYNDORME BY OTHER M.D. ON MARCH 19, 2001. THIS DATE IS THE DATE OF INJURY.

DR. DIAGNOSED EE AS HAVING WORK-RELATED CARPAL TUNNEL SYNDROME. THE DIAGNOSIS WAS CONFIRNED BY UNITY CORPORATE HEALTH ON NOVEMBER 30, 2000. THEREFORE, THIS DATE IS THE DATE OF INJURY.

EE, TEAMSTER WAS DIAGNOSED AS HAVING WORK-RELATED LEFT CARPAL TUNNEL SYNDROME BY M.D. THE DIAGNOSID WAS CONFIRMED BY UNITY CORPORATE HEALTH BY NOV 21, 2000. THIS DATE IS THE DATE OF INJURY.

WHILE BEING TREATED FOR CARPAL TUNNEL SYNDROME (MSHA FORM 7000 DATED 12/04/00) THE EMPLOYEE HADTHIS EARLIER NON-RPORTABLE INJURY RE-EVALUATED, RESULTING IN A SURGERY DIAGNOSIS ON 1/17/01. EE WAS LOADING WHEEL BEARINGS FOR A 50-TON HAUL TRUCK INTO A PICK-UP TRUCK FOR DELIVERY TO THE PLANT. THE BEARINGS WEIGH APPROXIMATELY 50 TO 60POUNDS.

EMPLOYEE WAS MOUNTING THE CATERPILLAR 777D HAUL TRUCK AFTER HIS LUNCH BREAK. WHEN HE REACHED UP TO GRAB THE HAND RAIL MOUNTED BELOW THE CAB WINDSHIELD, HE FELT PAIN OR TEAR IN HIS LEFT SHOULDE R AND UPPER ARM.

EE WAS PERFORMING CLEAN UP AT SECONDARY PLANT WHEN HE WAS ASKED TO HELP IN THE REPLACEMENT OF CURTIN LINERS IN THE IMPACT CRUSHER. EE STATED HE LIFTED ONE OF THE CURTIN LINERS 10" X 15" X 3", APPROX 130 LBS, BY HIMSELF UP TO THE MACHINIST. HE DID NOT NOTICE ANY IMMEDIATE PAIN. LATER THAT EVENING & NEXT MORNING EXPERIENCED PAIN. 0N 10/12/99 DIAGNOSED COMPRESSION FRACTURE OF SPINE.

EE WAS HOLDING A MAUL WHICH WAS BEING STRUCK BY A HAMMER TO REMOVE A STABILIZER PIN FROM A D9L DOZER. A PIECE OF METAL WAS DISLODGED BY THE IMPACT AND STRUCK THE EE IN THE UPPER LEFT ARM, PENE TRATING TWO LAYERS OF CLOTHING AND LODGING 1/4" TO 3/4" BENEATH THE SKIN. EE WAS TAKEN TO UNITY HEALTH CLINIC AND THEN TO A PLASTIC SURGEON, WHO REMOVED THE OBJECT FROM THE ARM AT HIS OFFICE.

UNLOADING TRAILER HE SLIPPED AND FELL FACE FIRST BREAKING HIS TOOTH.

EE WAS SHOVELING MATEIRAL FROM THE TUNNEL LEADING FROM THE CRUSHER. HE WAS WEARING A SUPPORT BACK BRACE. HE FELT HIS BACK INJURY AT THE TIME BUT THOUGHT IT WAS JUST A STRAIN. IT DID NOT IMPROV E AND HE WENT TO THE HOSPITAL ON 4-29-94.

CHANGING OIL ON A 992 FRONT END LOADER JAMMED FINGER INTO A TIRE WHILE DISMOUNTING THE LOADER CAUSING A BREAK TO FINGER

WHILE CLEANING A BIN UNDER #3 SCREEN, MATERIAL FELL CATCHING THE EMPLOYEE IN THE BIN AND PROHIB ITING MOVEMENT FROM THE KNEES DOWN. EMPLOYEE WASTRANSPORTED TO THE HOSPITAL WHERE NO INJURY WAS FOUND TO HAVE BEEN SUSTAINED AND HE WAS RELEASED

WHILE PUTTING A HUB ON A CATEPILLAR 50-TON TRUCK HE PULLED HIS RIGHT THIGH MUSCLE.

EMPLOYEE WAS OPENING GATE ON RAIP RAP CHUTE. THE GATE SLIPPED AND CUT THUMB ON EMPLOYEE'S RIGHT HAND.

DURING MSHA INSPECTION, OUR SAFETY DIRECTOR WAS DESCENDING TO LOWER LEVEL BY LADDER. HIS FEET SLIPPED OFF THE RUNGS AND HE CAUGHT HIMSELF BY GRABBING A RUNG. HIS WEIGHT CAUSED HIS LEFT SHOUL DER TO DISLOCATE.

WHILE GREASING THE PLANT FELL OFF CONVEYOR INJURING HIS BACK. AT THE TIME HE REFUSED MEDICAL ATTENTION BUT WAS TAKEN TO HOSPITAL FOR X-RAYS.

WHILE CLEANING 4ATER OFF THE FLOOR OF THE SHOP SEMPLOYEE SLIPPED AND FELL INJURING HIS BACK

WHILE WELDING ON HAMMERS, TILLEY RECEIVED FLASH BURNS OF THE EYES. HE WAS WEARING WELDING GLASSES (SHIELD).
