Employee was trying to remove an impeller from the shaft when the chain wrench came loose causing employee to fall backwards into a 12"" discharge pip. The flange hit in the center of the employees back causing injury.

I was servicing the dredge engine. You have to step up on top of a welder to service the engine. As I was stepping down off the engine I stepped onto a piece of 2"" pipe and rolled my ankle spraining it.
