Employee was trying to remove an impeller from the shaft when the chain wrench came loose causing employee to fall backwards into a 12" discharge pip. The flange hit in the center of the employees back causing injury.

I was servicing the dredge engine. You have to step up on top of a welder to service the engine. As I was stepping down off the engine I stepped onto a piece of 2" pipe and rolled my ankle spraining it.

Two workers were replacing the tail pulley on Extec Conveyor. They were pulling pulley over the rock deflector when injured employee's end dropped and pinched his finger between the pulley shaft and the frame.
