WHILE HIS 777 END DUMP TRUCK WAS BEING LOADED, EE REACHED BEHIND THE SEAT FOR PAPER TOWELS AND STUCK HIS FINGER IN THE HEATER VENT. HE WAS TAKEN TO THE LOCAL E.R. WHERE HE RECEIVED 5 STITCHES AND WAS GIVEN A PRESCRIPTION DORVOCET AND RELEASED TO WORK AT RESTRICTED DUTY.

AT 0530 LOADER #I-62 CAUGHT ON FIRE. 5 HANDHELD FIRE EXT. WERE USED BUT WERE UNSUCCESSFUL. THE LOCAL FIRE DEPT RESPONDED & THE FIRE WAS EDXTINGUISHED AT APPROX 0615. THIS WAS REPORTED TO MSHA AT 0630 AND THE SCENE PRESERVED. CAUSE OF FIRE UNKNOWN AT THIS TIME. DAMAGES: TOTAL LOSS.

WHILE CLIMBING THE LADDER OF A 777 END DUMP TRUCK, EE LOST HIS GRIP AND FELL. HE WAS TAKEN TO THE LOCAL E.R. WHERE HE WAS DIAGNOSED WITH A FRACTURED UNLNE AND RADIUS OF HIS RIGHT ARM. CONTIBUT ING FACTORS-WEATHER-COLD AND WET.
